Impacto de um programa de comunicação entre os enfermeiros na ocorrência de eventos adversos e na adoção de práticas de segurança

Entretanto, existem situações que predispõem ao risco de eventos adversos, que são incidentes ou circunstâncias decorrentes do cuidado assistencial à saúde e que resultam em dano não associado à doença de base do paciente. O objetivo geral do estudo foi avaliar a ocorrência de eventos adversos e práticas de segurança após um programa de intervenção de comunicação na passagem de plantão em um hospital público de Porto Alegre. Este estudo caracteriza-se por ser um estudo de intervenção quase-experimento, do tipo antes e depois. O estudo realizou-se nas unidades de internação do Hospital Independência, de Porto Alegre, RS. Os desfechos de interesse coletados foram eventos adversos e práticas de segurança específicos: incidência de atraso da primeira dose do antibiótico em pacientes com infecção; tempo de permanência do cateter venoso central; tempo de permanência do cateter vesical; tempo entre a solicitação e realização de ecografia e tomografia. A coleta dessas variáveis foi realizada de 07 de dezembro de 2015 a 07 de março de 2016 no período pré-intervenção. No mês subsequente todos os enfermeiros receberam treinamento de como utilizar a ferramenta na passagem de plantão. De 10 de abril a 10 de julho de 2016 (período pós-intervenção) foram novamente coletadas as variáveis. No período pré-intervenção, houve 709 internações no hospital. No período pós-intervenção, houve 871 internações. Houve redução de 56% no tempo entre a prescrição e a administração da primeira dose de antibiótico no período pós-intervenção. Não houve diferença nos dias de permanência de cateter venoso central, nem de sonda vesical de demora. O tempo médio entre a solicitação e a realização de ecografia passou de 37,8 ± 26,4 horas no período pré-intervenção para 17,6 ± 15,8 horas no período pós-intervenção (p = 0,01). Com relação à tomografia, a mediana do tempo entre a solicitação e a realização do exame passou de 12,0 (4,0 – 20,0) horas no período pré-intervenção para 4,0 (1,0 – 18,7) horas no período pós-intervenção (p = 0,02). Nós concluímos que a utilização de uma ferramenta de comunicação na passagem de plantão da enfermagem associou-se à redução no tempo de início de antibiótico e no tempo de espera para a realização de ecografia e tomografia, sugerindo que uma comunicação efetiva no momento da passagem de plantão contribui para maior adoção de práticas de segurança e redução de eventos adversos.Health institutions have as a basic principle in providing services to patients with the minimum or total absence of risks and failures that could compromise patient safety. However, there are situations that predispose to the risk of adverse events, which are incidents or circumstances resulting from health care and that result in damage not associated with the patient's underlying disease. The general objective of the study was to evaluate the occurrence of adverse events and safety practices after a communication intervention program during change of shift in a public hospital in Porto Alegre. This study is characterized by being a quasi-experimental intervention study, of the before and after type. The study was carried out in the hospitalization units of the Hospital Independência, Porto Alegre, RS. The outcomes of interest collected were specific adverse events and safety practices: incidence of delay of the first dose of the antibiotic in patients with infection; duration of the central venous catheter; duration of the urinary catheter; and time between the request and the accomplishment of ultrasound and tomography. The collection of these variables was performed from December 7, 2015 to March 7, 2016 in the pre-intervention period. In the following month all the nurses received training on how to use the tool during change of shift. From 10 April to 10 July 2016 (postintervention period) the variables were again collected. In the pre-intervention period, there were 709 hospital admissions. In the post-intervention period, there were 871 admissions. There was a 56% reduction in the time between the prescription and administration of the first dose of antibiotic in the postintervention period. There was no difference in the duration of central venous catheter stay nor in duration of urinary catheter. The mean time between the the request and the accomplishment of ultrasound was 37.8 ± 26.4 hours in the pre-intervention period and 17.6 ± 15.8 hours in the post-intervention period (p = 0.01). Regarding tomography, the median time between the request and the accomplishment was changed from 12.0 (4.0 - 20.0) hours in the pre-intervention period to 4.0 (1.0 - 18.7) hours in the postintervention period (p = 0.02). We conclude that the use of a communication tool in the nursing shift was associated with the reduction in the time of antibiotic start and the waiting time for ultrasound and tomography, suggesting that effective communication during change of shift contributes to greater adoption of safety practices and reduction of adverse events
