Avaliação da neurofisiologia da dor em filhas de pacientes fibromiálgicas: estudo caso-controle

No. of bitstreams: 1 rjmuniz.pdf: 2023919 bytes, checksum: 1c6d04dcb3725871b82fc2b82d285568 (MD5) Previous issue date: 2015A síndrome da fibromialgia é caracterizada por dor crônica generalizada, está relacionada ao sistema musculoesquelético, causando hiperalgesia e alodinia associada a alterações neuronais nas vias descendentes da dor, sensações de fadiga, comportamentos tipo ansiosos e depressivos e altos níveis de catastrofismo. Objetivou-se compreender a relação entre o processo neurofisiológico da dor na fibromialgia e seus potenciais marcadores de susceptibilidade à síndrome. Como objetivos secundários, buscou-se comparar o pensamento catastrófico da dor em filhas de mulheres fibromiálgicas comparadas com filhas de mulheres sem a síndrome (artigo1), verificar a eficácia do CPM como medida do sistema inibitório descendente da dor em filhas de mulheres fibromiálgicas (artigo2), avaliar os níveis de atividade física em mulheres filhas de pacientes fibromiálgicas (artigo3). O estudo foi aprovado pelo Comitê de Ética e Pesquisa do Hospital de Clínicas de Porto Alegre-RS (HCPA). Trata-se de estudo caso-controle, realizado no Laboratório de Dor e Neuromodulação do HCPA. Foram recrutados 76 mulheres, com idade superior a 18 anos. Todas assinaram o Termo de Consentimento Livre e Esclarecido (TCLE). Foram utilizados os seguintes instrumentos: Questionário Sócio-Demográfico, Escala de Ansiedade Traço-Estado (STAI), Escala de Catastrofismo, Escala de Resiliência, Inventário de Depressão de Beck (BDI–II), Questionário Internacional de Atividade Física (IPAQ), Escala Funcional da Dor, Teste Sensorial Quantitativo (QST), a Modulação da Dor Condicionada (CPM), Limiar de Dor à Pressão (Somedic AB, Stockholm, Sweden) e níveis séricos de BDNF e estradiol. Os dados foram analisados no programa SPSS versão 20.0. Os resultados demonstraram diferença significativa no perfil da dor crônica, catastrofismo total, comportamento do tipo depressivo e estado de ansiedade e níveis de atividade física. Isso demonstrou que as filhas de pacientes com fibromialgia apresentam pensamentos catastróficos em relação à dor e comportamento do tipo depressivo, praticam menos atividade física. Níveis séricos de BDNF e estradiol não apresentaram diferença significativa. Na avaliação do CPM, as mulheres do grupo caso apresentaram um sistema inibitório comprometido, sugerindo que alterações fisiológicas no sistema de controle da dor, mas sem alterações em biomarcadores periféricos, como o BDNF. Portanto, filhas de pacientes fibromiálgicas apresentam alterações emocionais, comportamentais, funcionais e alterações nas vias descendentes da dor. O CPM é eficaz e pode ser explorado na avaliação do sistema descendente da dor endógena. Estudos de coorte podem ser utilizados para avaliar possíveis alterações em filhas de pacientes fibromiálgicas, avaliando a saúde psicológica e funcional, na busca por medidas preventivas.Fibromyalgia syndrome is characterized by chronic widespread pain and it is related to the musculoskeletal system, causing hyperalgesia, allodynia associated with neuronal changes in the descending pathways of pain, fatigue, feelings such as anxiety and depression and high levels of catastrophism. The objective of this study was to understand the relationship between the neurophysiological process of pain in fibromyalgia and their potential susceptibility markers of the syndrome. The secondary objective was to compare the catastrophic thinking of pain in daughters of women diagnosed with fybromialgia, compared to daughters of women without the syndrome (Article 1), verify the effectiveness of CPM as a measure of the inhibitory system of pain in daughters of women with fibromyalgia (Article 2) and analyze physical activity levels in daughters of fibromyalgic women (Article 3). The study was approved by the Ethics and Research Committee of Hospital de Clinicas de Porto Alegre-RS (HCPA). It was a case-control study, conducted at the Pain and Neuromodulation Laboratory of HCPA. 76 women older than 18 years were recruted. All of them signed the Informed Consent (IC). The following instruments were used: Socio-Demographic Questionnaire, the State-Trait Anxiety Scale (STAI), Catastrophism Scale, Resilience Scale, Beck Depression Inventory (BDI-II), International Physical Activity Questionnaire (IPAQ), Quantitative Scale Functional Pain, Sensory Test (QST), the Conditioned Pain Modulation (CPM), Pain Threshold to Pressure (Somedic AB, Stockholm, Sweden) and serum levels of BDNF and estradiol. Data was analyzed using SPSS version 20.0. The results showed significant differences in the type of chronic pain, full catastrophism, depression and anxiety and physical activity levels. This demonstrated that the daughters of women who were fibromyalgia patients already had catastrophic thoughts about the pain and depressive-like behavior, practiced less physical activity. Serum BDNF and estradiol showed no significant difference. In the CPM evaluation, the group case presented a compromised inhibitory system, suggesting that physiological changes in the pain control system were present, but there were no changes in peripheral biomarkers, such as BDNF. Therefore, the daughters of fibromyalgic patients already have an altered emotional, behavioral and functional state, and changes in descending pathways of pain. The CPM is effective and can be useful to evaluate the system of the descending endogenous pain. Cohort studies can be used to analyze possible changes in daughters of fibromyalgic patients, evaluating their psychological and functional health, searching for preventive measures
