The experience of illness from the perspective of people who survived the stroke.

The experience of experiencing stroke can lead the person and family caregivers to face the demands arising from dysfunctions resulting from this condition. Currently there is a broad, multidisciplinary scientific debate about the advances necessary to promote health and quality of life to people who survive stroke, through health care models that use a comprehensive approach of health, with emphasis on rehabilitation process. The aim of the study was to analyze the meanings of the illness experience from the perspective of people who survived the stroke. This is a qualitative case study, which involved eight adult and elderly, treated at a general hospital in the south of Bahia, from January 2011 to December 2012. Data were collected through semi-structured interviews in houses and the interpretive analysis conducted through the stages of reduction, organization of data, identification of meaning units, construction of core themes and interpretation of results. The experience of the disease was understood as a biographical disruption, a situation between life and death, disability, loss of autonomy and inability to work. The social support from family and religion were essential to deal with the changes in everyday life after stroke. Needs related to rehabilitation interventions in the home and community context were identified, and the participants mentioned difficulties in the process of rehabilitation after discharge. The stroke experience marked a break in the lifestyle and the way people work and understand the disease. The absence of a program of rehabilitation and upgrading is a clear distortion between the ideal employee for public policy and the actual neglect of the poor service that contributes to the dehumanization of health care. Participants and their families were left without support for treatment and continuity of care network. It became apparent dismantling of the organizational structure of health care networks, and their unpreparedness to meet the demands of health care to patients with chronic diseases, especially in inner cities whose human and technological resources are scarce. These results suggest the need to enhance the strategies of educational interventions and multidisciplinary care, as well as revising the process of self-management of health and to strengthen the network of social support, especially from family that will meet the demands of care associated with population aging and coping with chronic diseases, especially stroke.A recuperação após o acidente vascular cerebral (AVC) é complexa e envolve aspectos biomédicos, sociais e psicológicos associados à saúde e a qualidade de vida. A experiência de vivenciar o acidente vascular cerebral pode levar a pessoa e seu cuidador familiar a enfrentar as exigências decorrentes das disfuncionalidades resultantes dessa condição. Atualmente observa-se, um debate científico amplo e multidisciplinar sobre os avanços necessários para promover a saúde e a qualidade de vida das pessoas que sobrevivem ao AVC, por meio de modelos de atenção a saúde que utilizem uma abordagem abrangente de saúde, com ênfase no processo de reabilitação. O objetivo do estudo foi analisar os sentidos e significados da experiência da enfermidade na perspectiva de pessoas que sobreviveram ao acidente vascular cerebral. Trata-se de um estudo de caso qualitativo, do qual participaram oito pessoas adultas e idosas, atendidos em um hospital geral do sudoeste da Bahia, no período de janeiro de 2011 à dezembro de 2012. Os dados foram coletados por meio de entrevista semiestruturada nos domicílios e a análise interpretativa conduzida por meio das etapas de redução, organização dos dados, identificação das unidades de significado, construção dos núcleos temáticos e interpretação dos resultados. A experiência da enfermidade foi compreendida como uma ruptura biográfica, situação entre a vida e a morte, invalidez, perda de autonomia e incapacidade para o trabalho. O apoio social da família e da religião foi essencial para lidar com as mudanças na vida cotidiana após acidente vascular cerebral. As necessidades relacionadas com intervenções de reabilitação no contexto domiciliar e comunitário foram identificados, e os participantes mencionaram dificuldades para o processo de reabilitação após a alta hospitalar. A experiência AVC marcou uma ruptura no estilo de vida e a forma como as pessoas trabalham e entendem a doença. A ausência de um programa de reabilitação e readaptação representa uma nítida distorção entre o ideal empregado pelas políticas públicas e o real descaso com o atendimento precário que contribui para a desumanização do cuidado em saúde. Os participantes e familiares ficaram sem rede de apoio para o tratamento e continuidade dos cuidados. Ficou evidente a desarticulação da estrutura organizacional das redes de atenção à saúde e seu despreparo para atender as demandas de cuidados de saúde aos pacientes com doenças crônicas, especialmente em cidades do interior cujos recursos humanos e tecnológicos são escassos. Esses resultados sugerem a necessidade de aprimorar as estratégias de intervenções educativas e atendimento multiprofissional, assim como, rever o processo de autogestão da saúde bem como fortalecer a rede de apoio social, especialmente da família que irá enfrentar as demandas de cuidado associado com o envelhecimento populacional e ao enfrentamento das enfermidades crônicas, especialmente o acidente vascular cerebral
