Lezarea structurilor inelului pelvian în timpul naşterii la multipare

The results of the treatment of unstable pelvic ring fractures in women in 11-64% of cases are unsatisfactory. There are sporadic publications about patients with pelvic instability after birth. Postpartum women ignore pain in the lumbo-pelvic region, "duck walking", urinary disorders. Objective of the study. Improving the course of pregnant women with pelvic ring lesions during childbirth, due to mandatory orthopedic examination. Material and Methods. Analyzed the results of treatment 5 women with diastase of the pubic symphysis, after repeated births. Performed clinical examination, radiography, etc. Radiological examination determined diastase between the pubic bones 14 - 46 mm, and in 3 patients the ilio-sacral junction was enlarged to 8-12 mm. Study funded by NRDA, project no.20.80009.8007.11, 20.800009.8007. Results. In 3 patients performed reconstructive pelvic surgery in the early stages of labor, in 1 stage, in 2 patients the treatment in stages using osteoplasty, combined osteosynthesis. After reconstructive interventions, the patients noticed an improvement in the general condition, the persistent pain disappeared, the gait improved, the claudication disappeared. Patients returned to their previous level of physical activity within 4-6 months. Radiological control of the pelvis confirmed the effectiveness of the treatment; formed the consolidation, maintaining the stability of the metal structure. Results within 3-8 years: the value on the scale S.A. Majeed is 80-94 points. Conclusion. Ignoring orthopedic consultations by women after the first birth caused the formation of a more significant diastase of the pubic bones in subsequent births. Reconstructive interventions performed on an unstable pelvic ring give a positive result and is a method of choice.Introducere. Rezultatele tratamentului fracturilor instabile ale inelului pelvian la femei în 11-64% sunt nesatisfăcătoare. Există publicații sporadice despre pacientele care suferă de instabilitate pelvină după naștere. Femeile postpartum ignoră durerile în regiunea pelviană, „mersul raței” etc. Scopul lucrării. Îmbunătățirea conduitei gravidelor cu leziuni ale inelului pelvian în timpul nașterii, datorită examinării ortopedice obligatorie. Material și Metode. Sunt analizate rezultatele tratamentului la 5 femei cu diastaza simfizei pubiane după nașteri repetate. A fost efectuat examenul clinic, de laborator și radiografie. Radiografiile au determinat diastaza între oasele pubiene 14-46 mm, iar la 3 paciente a fost mărită fanta joncțiunii ilio-sacrale la 8-12 mm. Studiul finanţat ANCD, proiectul nr.20.80009.8007.11, 20.800009.8007. Rezultate. Au fost efectuate intervenții reconstructive pe bazin în stadiul incipient al travaliului, într-o etapă – la 3 paciente, la 2 paciente – tratamentul în etape utilizând osteoplastia, osteosinteză combinată. După intervențiile reconstructive, pacientele au remarcat îmbunătățirea stării generale, durerea persistentă a dispărut, mersul s-a îmbunătățit. Pacientele au revenit la nivelul lor anterior de activitate fizică în decurs de 4-6 luni. Radiografia de control a bazinului a confirmat eficacitatea tratamentului – a fost formată consolidarea, menținerea stabilității structurii metalice. Rezultatele în decurs de 3-8 ani: valoarea cantitativă pe scara S.A. Majeed constituie 80-94 puncte. Concluzii. Ignorarea consultațiilor ortopedice de către femei după prima naștere a provocat formarea unei diastaze mai semnificative a oaselor pubiene la nașterile ulterioare. Intervențiile reconstructive efectuate pe un inel pelvian instabil dă un rezultat pozitiv și este o metoda de alegere
