موانع دسترسی به خدمات پزشکی اورژانس در بافت قدیم شهر سمنان

Introduction: One of the basic criteria in improving the level of public health is to increase the level of citizens' access to medical services, especially emergency services. The old districts of cities have many problems including problems in accessing medical services. The aim of this study is to investigate the obstacles to having access to emergency medical services in a standard time in the historical area of Semnan. Methods: The present research was an applied descriptive field study, which was performed in the Emergency Medical Center (115 Emergency) of Semnan using interviews and questionnaires. The study population in determining the obstacles included the staff and the director of Semnan Emergency Medical Center. The obstacle confirmation phase, included 15 experts who were selected via convenience sampling. First, based on library studies, previous information, consultation with 115 emergency personnel of Semnan, and field inspection, the indicators affecting accessibility to 115 emergency medical services in the old area of Semnan were extracted. The extracted indicators were assessed using Delphi method and a questionnaire was prepared using these indicators, based on the pairwise comparison matrix of the present study. The prepared questionnaire was distributed and explanations were given on how to answer the questions and they were collected after being filled out. The data obtained from the questionnaire were entered into decision making trial and evaluation laboratory (DEMATEL) software and were analyzed using Dematel technique.Results: The most important barriers to accessing emergency medical services in the studied area, from the perspective of research experts, were found to be transportation standards with a weight of (35/318), architecture with a weight of (35/163), education with a weight of (34/778), time with a weight of (34/665), and communication with a weight of (34/238), respectively.Conclusion: 3 of the 5 criteria constituting barriers to access to emergency medical services were of the effective type and only two criteria of transportation and communication were of the affected type. Therefore, these factors have a high weight and are confirmed as barriers to accessing emergency medical services in the studied area. Also, considering that there were no evaluation errors (including lack of supervision) in the model of this research, it can be said that these factors are the most important barriers to accessing emergency medical services.مقدمه: یکی از معیارهای اساسی در ارتقاء سطح بهداشت جامعه، افزایش میزان دسترسی شهروندان به خدمات درمانی به ویژه خدمات اورژانس است. بافت‌های قدیمی و فرسوده شهری مشکلات متعددی از جمله دسترسی به خدمات بهداشتی- درمانی دارند. هدف این پژوهش، بررسی موانع دسترسی به خدمات پزشکی اورژانس در زمان استاندارد در بافت تاریخی شهر سمنان رخ داده است. روش کار: پژوهش حاضر از نوع کاربردی و توصیفی-میدانی بود که با استفاده از مصاحبه و پرسش‌نامه در مرکز فوریت‌های پزشکی (اورژانس 115) شهر سمنان انجام گرفته است. جامعه آماری در تعیین موانع شامل کارکنان و مدیر مرکز فوریت‌های پزشکی شهر سمنان بود. مرحله تایید موانع شامل 15 نفر از افراد مطلع بودند که بر اساس در دسترس بودن انتخاب گردیدند. ابتدا با توجه به مطالعات کتابخانه‌ای، اطلاعات قبلی، هم‌اندیشی با پرسنل اورژانس 115 سمنان و بازدید میدانی، شاخص‌های موثر بر موانع دسترسی به خدمات پزشکی اورژانس 115 در بافت قدیم شهر سمنان استخراج گردید. شاخص‌های بدست آمده را با استفاده از روش دلفی مورد بررسی قرار داده و با استفاده از این شاخص‌ها پرسش‌نامه بر اساس ماتریس مقایسات زوجی پژوهش حاضر تهیه شد. پرسش‌نامه تهیه ‌شده توزیع و پس از توضیح در خصوص نحوه پاسخگویی به سوالات و تکمیل آن، جمع‌آوری گردید. داده‌های حاصل از پرسش‌نامه وارد نرم افزار دیماتل گردید و با استفاده از تکنیک دیماتل مورد تحلیل قرار گرفت. یافته‌ها: مهمترین موانع دسترسی به خدمات پزشکی اورژانس، از دیدگاه افراد متخصص در حوزه تحقیق در منطقه مورد مطالعه به ترتیب معیارهای حمل‌ونقل با وزن (318/35)، بافت با وزن (163/35)، آموزش با وزن (778/34)، زمان با وزن (665/34) و ارتباطات با وزن (238/34) شناسایی شدند. نتیجه گیری: از مجموع 5 معیار تشکیل دهنده موانع دسترسی به خدمات پزشکی اورژانس 3 معیار از نوع اثر‌گذار بوده و تنها دو معیار حمل‌ونقل و ارتباطات از نوع اثرپذیر بوده‌اند. لذا این عوامل درصد بالایی را به خود اختصاص داده و بعنوان موانع دسترسی به خدمات پزشکی اورژانس در منطقه مورد مطالعه تحقیق تایید می‌شوند. همچنین با توجه به اینکه در مدل این تحقیق خطاهای ارزیابی (از جمله نبود نظارت) وجود نداشته است، لذا می‌توان گفت این عوامل مهم‌ترین موانع دسترسی به خدمات پزشکی اورژانس هستند
