اندازه گیری سطح فیبرینوژن در بیماران مبتلا به ترومای لگن

Pelvic fracture is the second most common cause of death in trauma patients after head trauma; and the major reason for death in these patients is hemorrhage and its resulting hypovolemic shock. Rapid and proper diagnosis and treatment can improve the outcome of these patients. The main treatment consists of fixation of the pelvis and resuscitation of patient’s hemodynamic using isotonic liquids and blood products until the condition is favorable for surgery or angiography. New findings show that fibrinogen plays a vital role in reaching and maintaining homeostasis, especially in patients that have lost a significant amount of blood. It seems that concentration of plasma fibrinogen, which is a key glycoprotein in the coagulation cascade, is independently related to hemorrhage and prophylaxis infusion of fibrinogen concentrate can decrease bleeding. Fibrinogen level has been introduced as a determining factor in prediction of 28-day mortality of patients with heavy traumatic bleeding. Compared to fresh frozen plasma (FFP), injection of fibrinogen concentrate has had better results in improvement of outcome and has decreased need for other blood components and has led to an increased probability of survival without increasing the risk of venous thromboembolism. A study in 2015 showed that 19.2% of the patients with severe hemorrhage due to trauma had a fibrinogen level less than 2gr/L.Writers of the current letter evaluated 60 patients with traumatic pelvic fracture with the mean age of 51.12 ± 20.30 years (11-85) (58.30% male). 42 (70.0%) cases had presented with 1-sided or double sided ramus fracture, 17 (28.3%) cases with iliac wing fracture and 1 (1.7%) case with acetabulum fracture. 16.7% of the patients had a hemoglobin level less than 10 mg/dL and 21.7% had systolic blood pressure less than 90 mmHg on admission to emergency department. Mean blood level of fibrinogen in this group of patients was 261.85 ± 40.24 mg/dL (200-368). In other words, considering the normal range described for the kit used for this study (Mahsa Yaran Company, Iran) none of the patients were in the hypofibrinogenemia zone (< 200 mg/dL). It seems that variables such as trauma severity, racial characteristics, underlying illnesses, hemorrhage severity, type of fracture, and … might also affect the level of plasma fibrinogen and its decrease following hemorrhage. Overall, a final decision regarding the effect of fibrinogen injection instead of products such as FFP requires further studies with higher accuracy and considering other factors affecting the outcome of such patients. However, considering the studies that affirm the role of fibrinogen in improving the outcome of patients with severe hemorrhage, it seems that measuring plasma fibrinogen level and injecting it for patients with hypofibrinogenemia is a wise solution in management of this type of patients.شکستگی لگن بعد از ترومای سر شایع ترین علت مرگ در بیماران ترومایی می باشد و علت اصلی مرگ در این بیماران، خون ریزی و شوک هیپو ولمیک ناشی از آن است. تشخیص و درمان سریع و مناسب، می تواند در بهبود پیامد نهایی این گونه بیماران تاثیر گذار باشد. درمان اصلی شامل فیکس کردن لگن و احیائ همودینامیک بیمار با استفاده از مایعات ایزوتونیک و فراورده های خونی تا فراهم شدن شرایط لازم برای عمل جراحی یا آنژیوگرافی است. یافته‌های جدید نشان می‌دهند که فیبرینوژن نقشی حیاتی در دستیابی و نگهداری هموستاز، به ویژه در بیمارانی که به میزان قابل ملاحظه‌ای خون از دست داده اند، دارد. به نظر می‌رسد غلظت فیبرینوژن پلاسما که یک گلیکوپروتئین کلیدی در آبشار انعقادی است، به طور مستقل با خونریزی مرتبط باشد و انفوزیون پروفیلاکسی کنستانترة فیبرینوژن باعث کاهش خونریزی شود. سطح فیبرینوژن به عنوان یک عامل تعیین کننده در پیشگویی مرگ و میر 28 روزه بیماران مبتلا به خونریزی شدید ترماتیک معرفی شده است. تزریق کنستانتره فیبرینوژن در مقایسه با FFP نتایج بهتری در بهبود پیامد و کاهش نیاز به سایر اجزای خونی داشته و موجب افزایش احتمال بقا بدون افزایش ریسک ترومبو آمبولی وریدی شده است. مطالعه ای در سال 2015 نشان داد که 2/19 درصد از بیماران دچار خونریزی شدید ناشی از تروما دارای فیبرینوژن زیر 2 گرم در لیتر بوده اند. نویسندگان این نامه 60 بیمار مبتلا به شکستگی تروماتیک لگن با میانگین سنی 30/20 ± 12/51 سال (85 – 11) را مورد بررسی قرار دادند (30/58 درصد مرد). 42 (0/70 درصد) مورد با شکستگی راموس یک یا دو طرفه، 17 (3/28 درصد) مورد با شکستگی بال ایلیاک و یک (7/1 درصد) مورد با شکستگی استابولوم مراجعه کرده بودند. 7/16 درصد بیماران هموگلوبین زیر 10 میلی گرم در دسی لیتر و 7/21 درصد فشار خون سیستولیک زیر 90 میلی مترجیوه در بدو ورود به بخش اورژانس داشتند. میانگین سطح خونی فیبرینوژن در این گروه از بیماران 24/40 ± 85/261 (368-200) میلی گرم در دسی لیتر بود. به عبارتی با توجه به رنج نرمال تعریف شده برای کیت مورد استفاده برای مطالعه (شرکت مهسا یاران) هیچ یک از بیماران در محدوده هیپوفیبرینوژنمی (زیر 200 میلی گرم در دسی لیتر) قرار نداشتند. به نظر می رسد شاید متغییر هایی نظیر شدت تروما، ویژگی های نژادی، بیماریهای زمینه ای، شدت خونریزی، نوع شکستگی و ... نیز در میزان فیبرینوژن پلاسما و کاهش آن به دنبال خونریزی تاثیر گذار باشند. در هر صورت اظهار نظر نهایی در مورد تاثیرات تزریق فیبرینوژن به جای فراورده هایی نظیر FFP نیازمند مطالعات بیشتر و دقیق تر با در نظر گرفتن سایر متغییر های تاثیر گذار در پیامد این گونه بیماران دارد. با این حال با توجه به مطالعات تایید کننده نقش فیبرینوژن در بهبود پیامد بیماران با خونریزی شدید، به نظر می رسد اندازه گیری سطح پلاسمایی فیبرینوژن و تزریق آن برای بیماران مبتلا به هیپوفیرینوژنمی، راهکار عاقلانه ای در مدیریت این گونه بیماران باشد
