تاثيرآموزش بر آگاهی و عملکرد پرستاران بخش اورژانس در زمینه تریاژ بیماران

Introduction: Emergency department as one of the most important wards of the hospital confronts with lots of referring patients. Timely service presentation in this ward depends on efficient and effective functions of its personnel. Thus, this study was aimed to evaluate the effect of education on the knowledge and practice of emergency department’s nurses in the patients’ triage field. Methods: In this cross-sectional study, the knowledge and practice of 50 nurses was evaluated before and after of 9 hours educational workshop regarding patients’ triage based on the emergency severity index (ESI). Persons who had at least six months work experience in the emergency department and did not participate in any triage workshop during the six years before starting the project were entered to the study. Data gathering was performed through preparing three questionnaires separately included demographic information as well as assessment of knowledge and practice. Evaluated demographic characteristics were age, gender, marital status, work history, academic degree, type of employment, work shift, and average of work shift weekly. To assess the knowledge and practice, two separate questionnaires were used that their reliability and validity were confirmed before. Data was analyzed using SPSS 16 and appropriate analytic tests. P<0.05 was considered as statistically significant. Results: The average knowledge scores of nurses reached from 7.5±2.1 to 14±1.6 (p= 0.001, r=0.49) after education. Also the average scores of participants increased from 31.8±9.9 to 69.7±8.1 (p= 0.001, r=0.87). There was no significant relationship between characteristics of nurses and their knowledge scores in six weeks after education (p>0.05). While it was seen between the work history of nursing (p=0.038), working in emergency department (p=0.001), as well as type of employment (p=0.019) and average scores of practice within six weeks after education. No significant relation was seen between academic degree, marital status, and gender of participants and average scores of knowledge and practice during six weeks after finishing the educational workshop. Conclusion: It seems that education has a positive effect on increasing the knowledge and practice of emergency department’s nurses regarding the triage of patients by ESI. Therefore, expanding the theoretical and practical courses of education is essential to improve the knowledge and quality of service presentation.مقدمه: بخش اورژانس به عنوان یکی از مهمترین بخش های هر بیمارستانی با انبوه بیماران مراجعه کننده رو به رو است. ارائه خدمات موثر در زمان مناسب در این بخش در سایه عملکرد کارا و موثر پرسنل آن امکان پذیر خواهد بود. لذا در این مطالعه بر آن شدیم تا تاثیر آموزش را بر میزان آگاهی و عملکرد پرستاران بخش اورژانس در زمینه تریاژ بررسی کنیم. روش کار: در این مطالعه توصیفی، میزان آگاهی و عملکرد 50 پرستار در زمینه تریاژ قبل و شش هفته بعد از برگزاری کارگاه آموزشی نه ساعته پیرامون تریاژ به سیستم شاخص میزان اورژانسی بودن (Emergency Severity Index) ارزیابی شد. افرادی که دارای حداقل شش ماه سابقه كار در بخش اورژانس بودند و طی شش ماه قبل از آغاز پژوهش هم درهیچ کارگاهی مربوط به تریاژ شرکت نکرده بودند وارد مطالعه شدند. جمع آوری اطلاعات از طریق تنظیم سه پرسشنامه جداگانه شامل: اطلاعات دموگرافیک، ارزیابی و ارزیابی عملکرد انجام شد. خصوصیات دموگرافیک مورد ارزیابی شامل سن، جنس، وضعیت تاهل، سابقه کار، مدرک تحصیلی، نوع استخدام، شیفت کاری و متوسط تعداد شیفت کاری در هفته بود. به منظور ارزیابی و عملکرد دو پرسشنامه مجزا که پایایی و روایی آنها مورد آزمون و تایید قرار گرفته بود، مورد استفاده قرار گرفت. اطلاعات با استفاده از نرم افزار SPSS نسخه 16 و تست های آماری متناسب مورد تجزیه و تحلیل قرار گرفت. سطح معنی دار کمتر از 05/0 در نظر گرفته شد. يافته ها: میانگین نمره پرستاران از 1/2±5/7 به 6/1±14 افزایش یافت .(p =0.001, r=0.049) در حیطه عملکرد نیز میانگین نمرات افراد شرکت کننده از 9/9±8/31 به 1/8±7/69 رسید (p =0.001, r= 0.87). بین ویژگیهای فردی پرستاران و نمره آنها شش هفته پس از آموزش هیچ ارتباط معناداری یافت نشد (p > 0.05). درحالیکه بین سابقه کار پرستاری (p = 0.038)، کار در بخش اورژانس (p = 0.001)، نوع استخدام (p=0.019) و میانگین نمرات عملکرد در فاصله زمانی شش هفته پس از آموزش رابطه معناداری وجود داشت. همچنین بین نوع مدرک تحصیلی، وضعیت تاهل و جنسیت افراد شرکت کننده و میانگین نمرات آگاهی و عملکرد در فاصله زمانی شش هفته بعد از اتمام کارگاه آموزشی ارتباط معناداری به دست نیامد. نتيجه گيری: بنظر می رسد که آموزش تاثیر مثبتی بر افزایش میزان آگاهی و عملکرد پرستاران در زمینه تریاژ بیماران به روش ESI داشته باشد. بنابراین گسترش دوره های آموزش تئوری و عملی تریاژ برای پرستاران شاغل در بخش اورژانس، جهت ارتقاء آگاهی و به تبع آن کیفیت ارائه خدمات، ضروری به نظر می رسد
