بیماری کبد چرب غیر الكلی و نارسایی‌های ناشی از آن از دیدگاه طب سنتی ایرانی و طب رایج

Background and Aim: The liver is the largest organ in the body that plays a major role in metabolism and detoxification of various substances. Fatty liver is known to be the most common cause of cirrhosis and liver cancer. People with fatty liver have an increased risk of cardiovascular disease. Materials and Methods: This research is a review study with a library method that has been done by examining the documentary sources of traditional medicine, modern medicine, banks and various existing databases. Findings: NAFLD refers to the presence of hepatic steatosis without the involvement of secondary causes of fat accumulation in the liver, (excessive alcohol consumption). The basis of fatty liver treatment is based on lifestyle modification, which includes proper nutrition and exercise. Patients are mostly asymptomatic. It is recommended to treat mild cases where the person is prone to developing fibrosis. From the perspective of traditional medicine, patients' symptoms range from asymptomatic to feeling tired, bored and upset on the right side of the abdomen. Although fatty liver is not specifically associated with a particular disease, the clinical symptoms and treatment are similar to several traditional diseases, including obstruction of intrahepatic ducts, liver dysfunction especially cold liver dysfunction and ascites. Liver intrahepatic ducts obstruction is caused by various factors and can occur in the convex or concave part of the liver. Ethical Considerations: In this research, honesty and trustworthiness, citation of the texts used avoidance of bias in inferring the texts or analyzes have been observed. Conclusion: Some drugs used to treat fatty liver in modern medicine include metformin, thiazolidinediones. In traditional treatment of liver dyspepsia, hot tempered food is prescribed orally and sometimes topically. In relation to the hepatic duct obstruction, if the obstruction is in convex part, the diuretics are used. If the obstruction is in concave part, laxatives are prescribed. In the treatment of early stages of cirrhosis, enema and clearance with combination of aloe vera and other herbal medicines.   Cite this article as: Afsharipour M. Medical History Journal 2020; 12(44): e4.زمینه و هدف: کبد بزرگ‌ترین عضو بدن است که در متابولیسم، سوخت ‌و ساز مواد مختلف و سم‌زدایی نقش اصلی را در بدن ایفا می‌کند. کبد چرب به عنوان شایع‌ترین علت سیروز و سرطان کبد شناخته می‌شود. به علاوه در مبتلایان به کبد چرب احتمال عوارض قلبی ـ عروقی هم افزایش می‌یابد. مواد و روش‌ها: این تحقیق مطالعه مروری و با روش کتابخانه‌ای است که با بررسی منابع مستند طب سنتی ایران، طب رایج، بانک‌ها و پایگاه‌های مختلف اطلاعاتی موجود انجام گرفته است. یافته‌ها: بیماری کبد چرب غیر الكلی به وجود استئاتوز کبدی بدون دخالت علل ثانویه تجمع چربی در کبد مثل مصرف زیاد الكل، اطلاق می‌گردد. اساس درمان کبد چرب بر پایه اصلاح سبک زندگی می‌باشد که شامل دو محور اصلی تغذیه مناسب و ورزش است. موارد خفیف بیماری که فرد مبتلا، مستعد پیشرفت فیبروز می‌باشد، درمان توصیه می‌شود. کبد چرب با چند بیماری طب سنتی از جمله سوءمزاج‌های سرد کبد، سوءالقنیه و استسقاء تشابه دارد. از دیدگاه طب سنتی نیز علائم بیماران شامل طیفی از بی‌علامتی تا احساس خستگی، كسالت و ناراحتی سمت راست شکم می‌باشد. سدّه ‌کبدی در اثر عوامل مختلفی به وجود می‌آید که شایع‌ترین علل آن خلط غلیظ است. سده می‌تواند در قسمت محدب یا مقعر کبدی ایجاد شود. ملاحظات اخلاقی: در تدوین این پژوهش، اصل امانتداری و صداقت استناد به متون مورد استفاده، رعایت اصالت منابع و پرهیز از جانبداری در استنباط از متون یا تحلیل‌ها رعایت شده است. نتیجه‌گیری: برخی داروهای مورد استفاده در درمان کبد چرب در پزشکی رایج عبارتند از: متفورمین، تیازولیدین دیون‌ها. در درمان سنتی سوء‌مزاج سرد کبدی تجویز ادویه با طبیعت گرم به صورت خوراکی و گاهی به صورت موضعی و تأکید بر مصرف بیشتر غذاهای با طبع گرم می‌باشد. در درمان سوءمزاج تر کبدی، داروها و ادویه‌جات گرم، توصیه به ورزش، رژیم و پاکسازی بدن از خلط بلغم توصیه می‌گردد. در رابطه با سده کبدی، اگر سده در محدب کبد باشد از مدرات استفاده می‌شود. اگر سده در مقعر کبد باشد، مسهل تجویز می‌گردد. در درمان سوءالقنیه باید از تولید مواد زائد در بدن جلوگیری شود، سپس ایارج تجویز می‌شود
