Effect of old myocardial infarction on the serum levels of selected biomarkers of myocardial injury and healing and association of chromogranin A with the findings of myocardial perfusion SPECT

Background: Several biomarkers have gained importance over the years in clinical cardiology, in the diagnosis, follow-up, or risk stratification of cardiovascular disease. Chromogranin A (CgA) is a soluble polypeptide stored inside and released from secretory granules of endocrine, neuronal, and other cell types (including cardiomyocytes) and seems to be a marker of overall neuroendocrine activity. Apart from CgA, neopterin, cytoskeleton fragment 19 (CYFRA 21-1), tissue polypeptide antigen (TPA), tissue polypeptide specific antigen (TPS), C-terminal telopeptide of type I collagen (ICTP), N-terminal propeptide of type I procollagen (PINP), N-terminal propeptide of type III procollagen (PIIINP) are novel biomarkers that may also reflect on myocardial injury and healing processes.Purpose: This study investigated the potential role of serum CgA, neopterin, ICTP, PINP, PIIINP, CYFRA, TPA, and TPS as biomarkers of myocardial injury and healing in patients with old myocardial infarction (MI) and examined for possible association between CgA and the findings of myocardial perfusion scintigraphy (MPS).Method: A population of 307 patients (202 males, 105 females; aged 65.5±10.0 years) was enrolled. The study group consisted of 118 individuals (38.4%) with a history of myocardial infarction (MI group) more than one year old; the remaining 189 (61.6%) had no known heart disease (control group). All patients were submitted to MPS by single-photon emission computed tomography (SPECT) with Tc-99m-Tetrofosmin and had peripheral blood sampling on the same day for biomarker measurement with radioimmunometric assays (RIA/IRMA). To assess the association between our set of depended variables [(1) any-type deficits (abnormal MPS); (2) reversible deficits (ischemia); (3) fixed deficits (scar); (4) the extent of ischemia; and (5) the extent of scar] and a number of predictors (gender, CgA, LVEF, MI, hypertension, diabetes, hyperlipidemia and medications) we employed univariate and multivariate regression analyses. To test whether a possible effect of old infarction on serum CgA is mediated by SPECT findings, we employed analysis of covariance (ANCOVA) for the same five distinct categories of left ventricular (LV) perfusion deficits as dichotomous predictors. Serum CgA was measured in all study participants and the other seven biomarkers in a subpopulation of 86 patients (55 males, 31 females; aged 64.7±10.6 years) that were divided into three subgroups: 41 without a history of heart disease (control group); 26 with a history of old MI and preserved LV ejection fraction (EF) (LVEF≥50%) (MIpEF group); and 19 with old MI and reduced EF (LVEF<50%) (MIrEF group). Multivariate and univariate analyses of variance (MANOVA, ANOVA) were performed to test for biomarker differences between the three subgroups and the Scheffe posthoc test to identify exact inter-subgroup differences. A two-tailed p-value <0.05 was considered statistically significant.Results: In all five MPS SPECT categories, the effect of age, gender, and LVEF on serum CgA were unconditional and statistically significant: women exhibited higher CgA levels than men (p=0.008 to 0.025), whereas increasing age and decreasing LVEF were associated with increasing CgA (all p<0.001). Conversely, no statistically significant differences in mean CgA levels were found between MI patients and normal controls with either abnormal MPS, scar, or ischemia, or their extent. In the subpopulation of 86 patients, there was a significant inter-subgroup difference for the biomarkers’ linear combination (all multivariate tests p≤0.027). Subsequent univariate analyses showed a significant difference for ICTP (p<0.001) and NP (p=0.012), while the posthoc analysis revealed that the MIrEF group exhibited significantly higher ICTP and neopterin levels to both the MIpEF (p=0.022 and p=0.014) and the controls (p<0.001 and p=0.014, respectively). Conclusion: Although serum CgA is significantly associated with age, gender, and LVEF in patients with an old MI, no association was found between CgA levels and either history of an old MI or with SPECT findings. The elevated serum CgA observed during in the acute and subacute phase of MI appears to be blunted in infarctions older than a year. The significant association of serum ICTP and neopterin with old MI with compromised LVEF suggests that the underlying processes related to myocardial healing are ongoing more than a year after the insult. Further studies in larger populations are needed to validate the role of these biomarkers in clinical cardiology.Γενικά: Αρκετοί βιοδείκτες έχουν ερευνηθεί για την χρησιμότητά τους στην κλινική καρδιολογία, στο πεδίο της διάγνωσης, της παρακολούθησης ή της διαστρωμάτωσης κινδύνου των καρδιαγγειακών παθήσεων. Μελετήθηκε επίσης η πιθανή συσχέτιση της CgA με τα ευρήματα σε τομογραφικό σπινθηρογράφημα αιμάτωσης μυοκαρδίου (ΣΑΜ).Μέθοδος: Η μελέτη περιλάμβανε 307 ασθενείς (202 άνδρες, 105 γυναίκες, ηλικίας 65,5±10,0 ετών). Η ομάδα 1 της μελέτης αποτελούνταν από 118 άτομα (38,4%) με ιστορικό εμφράγματος μυοκαρδίου (ΕΜ) τουλάχιστον ένα χρόνο πριν (ομάδα ΕΜΦ) και η ομάδα 2 από τα υπόλοιπα 189 άτομα (61,6%) χωρίς γνωστή καρδιακή νόσο (ομάδα ελέγχου-ΕΛ). Για την εκτίμηση της σχέσης μεταξύ των εξαρτημένων μεταβλητών του ΣΑΜ [(1) οποιοδήποτε έλλειμμα (παθολογικό ΣΑΜ), (2) αναστρέψιμα ελλείμματα (ισχαιμία), (3) σταθερά ελλείμματα (ουλή), (4) έκταση ισχαιμίας και (5) έκταση ουλής] και μιας σειράς προγνωστικών παραγόντων (φύλο, CgA, ΕΜ, ΚΕΑΚ, υπέρταση, διαβήτης, υπερλιπιδαιμία, φαρμακευτική αγωγή) εφαρμόστηκε μονομεταβλητή και πολυμεταβλητή λογιστική παλινδρόμηση. Για να ελεγχθεί αν η πιθανή επίδραση του παλαιού εμφράγματος επηρεάζεται από τα ευρήματα του ΣΑΜ, διενεργήθηκε έλεγχος συνδιακύμανσης (analysis of covariance - ANCOVA) για τις ίδιες πέντε κατηγορίες ελλειμμάτων αιμάτωσης, ως διχοτόμοι προγνωστικοί παράγοντες. Η CgA μετρήθηκε σε όλους τους ασθενείς, ενώ οι υπόλοιποι δείκτες (Neopterin, CYFRA 21-1, ICTP, PINP, PIIINP, TPA, TPS) μετρήθηκαν στους 86 πρώτους ασθενείς (55 άνδρες και 31 γυναίκες, ηλικίας 64,7±10,6 ετών), οι οποίοι αποτέλεσαν τον πληθυσμό μιας επιμέρους μελέτης (μελέτη ΒΙΟ). Σε αυτή τη μελέτη οι ασθενείς χωρίστηκαν σε 3 ομάδες: 19 με ιστορικό παλαιού ΕΜ και χαμηλό ΚΕΑΚ<50% (ομάδα 1-ΕΜχΚΕ), 26 με παλαιό ΕΜ και διατηρημένο ΚΕΑΚ≥50% (ομάδα 2- ΕΜδΚΕ) και 41 χωρίς ιστορικό καρδιακής νόσου που αποτέλεσαν την ομάδα 3-ΕΛ (ομάδα ελέγχου). Διενεργήθηκαν μονομεταβλητές και πολυμεταβλητές αναλύσεις διακύμανσης (MANOVA, ANOVA) και Scheffe posthoc τεστ για τον έλεγχο διαφορών των βιοδεικτών μεταξύ των ομάδων.
