Characterization of the natural human skin tension in vivo

La peau humaine est l’enveloppe protectrice du corps contre toute sorte d’influences externes : biologiques, thermiques, chimiques et mécaniques. Elle a une structure complexe qui évolue avec l’âge, avec le mode de vie et avec les différents facteurs de vie. Les études de caractérisation des propriétés de la peau disponibles dans la littérature ne se préoccupent pas de la tension cutanée et du comportement anisotrope de la peau in vivo. Ces derniers représentent des propriétés importantes de la peau particulièrement dans les domaines de la chirurgie, de la dermatologie et de la cosmétologie. Ces travaux de thèse, réalisés en collaboration avec les Laboratoires Clarins, ont pour objectif de caractériser qualitativement et quantitativement la tension naturelle de la peau humaine in vivo en identifiant les causes conduisant aux variations qui existent : physiologie des couches de la peau, structure et orientation du réseau de fibres, et zones du corps et du visage. L’étude est réalisée en deux étapes pour différentes populations définies par leur tranche d’âge. La première étape a permis de montrer les liens entre la tension cutanée mesurée en surface et l’organisation plus profonde du derme en utilisant plusieurs méthodes et techniques de caractérisation in vivo et non-invasives de la peau : mécanique, topographique, biochimique et optique. Il est montré que la tension naturelle de la peau est due en grande partie à la tension exercée par les fibres de collagène et d’élastine. Cette tension est influencée par leur qualité et leur orientation. Cette première étape a permis également de déterminer les directions principales de la tension cutanée au niveau de deux zones du corps l’avant-bras et la cuisse. La deuxième étape consiste à étendre l’étude de la tension de la peau à d’autres zones du corps et du visage : le décolleté, le ventre, la face antérieure du bras, la face postérieure du bras et la joue. Ce protocole a permis de déterminer et de cartographier les directions principales de la tension de la peau au niveau de ces zones. Il a permis également d’identifier et d’étudier le comportement de deux principales familles de lignes de tension : la famille des lignes parallèles aux lignes de Langer et la famille des lignes perpendiculaires aux lignes de Langer. Ces familles ont des comportements mécaniques différents et subissent l’effet de l’âge de manière également différente. Il en résulte un comportement anisotrope de la peau. Cette étude a permis d’étudier l’état de tension cutanée et l’état d’équilibre des forces de tension de la peau entre les familles de lignes de tension. Il a été montré que la première famille se caractérise par une tension et une résistance importante lorsque la peau est soumise à une sollicitation mécanique, et que la deuxième famille a la particularité de présenter une capacité de déformation importante lors d’une sollicitation mécanique qui permet à la peau de s’adapter.Human skin is the protective envelope of the body against all kinds of external influences: biological, thermal, chemical, and mechanical. It has a complex structure that changes with age, with lifestyle and with different life factors. The characterization studies of these properties of the skin carried out and available in the literature are limited to the evaluation of its elastic and viscoelastic properties without regards to the skin tension and the anisotropic behavior of the skin. However, the skin tension and the anisotropic behavior of human skin are important properties of the skin, particularly in the fields of surgery, dermatology, and cosmetology.The aim of this thesis work, carried out in collaboration with Clarins Laboratories, is to qualitatively and quantitatively characterize the natural tension of human skin in vivo by identifying the causes leading to the variations that exist: physiology of the skin layers, structure and orientation of the fibers network, and body areas. The study is carried out in two stages for different populations defined by their age.The first step allowed to show the links between skin tension measured at the surface and the deeper organization of the dermis using several methods and techniques for in vivo and non-invasive characterization of the skin: mechanical, topographic, biochemical, and optical. It has been shown that the natural skin tension is due to the tension exerted by the collagen and elastin fibers. This tension is influenced by their quality and orientation. This first step allowed also to determine the main directions of the skin tension in two areas of the body, the forearm and the thigh.The second step extends the study of skin tension to other areas of the body and face which are: the neckline, belly, front of the arm, back of the arm and cheek. This step allowed to determine and map the main directions of skin tension in these areas. It also identifies and studies the behavior of two main families of skin lines tension: the family of lines parallel to Langer lines and the family of lines perpendicular to Langer lines. These families have different mechanical behaviors and are also influenced by the age of the person. This results in an anisotropic behavior of the skin.This study allowed to study the state of skin tension and the state of equilibrium of the tension forces of the skin between the families of tension lines. It has been shown that the first family is characterized by a high tension and resistance during the stress of the skin, and that the second family has the particularity of a significant deformation during a mechanical stress allowing the skin to adapt
