Clinical and demographic predictors of symptoms of depression and anxiety in patients with spinal cord injury

INTRODUÇÃO: A Lesão Medular é uma condição cujas consequências podem impactar nos aspectos físico, emocional e social na vida da pessoa que a sofre, bem como na dinâmica das relações sociais que a permeiam. São observadas altas taxas de problemas psicológicos após a instalação de uma deficiência, incluindo depressão e transtornos de ansiedade, além de outras formas de comportamentos adaptativos e sofrimento psíquico. Assim, é importante compreender os preditores da depressão e da ansiedade, visto que esse conhecimento pode favorecer insights sobre os mecanismos dessas alterações de humor na LM. Ainda, a presença de depressão e ansiedade é um fator que influencia no processo de reabilitação, sendo importante esse entendimento para um planejamento terapêuticos mais eficientes, o que pode resultar na melhora da qualidade de vida. OBJETIVO: O objetivo principal deste estudo foi identificar a relação preditiva e multivariada entre diferentes variáveis sociodemográficas e clínicas dos sintomas de depressão e ansiedade em pacientes com lesão medular. MÉTODO: Este foi um estudo transversal. Dados sociodemográficos e clínicos foram extraídos de 556 pacientes com LM no momento da avaliação inicial, prévio ao tratamento de reabilitação intensiva do Instituto de Reabilitação Lucy Montoro. Para identificar os fatores associados, realizamos modelos de regressão univariada e multivariada independentes através da Escala Hospitalar de Ansiedade e Depressão (HADS). RESULTADOS: Nossos resultados mostraram que os sintomas de depressão e ansiedade presentes nos pacientes acometidos por uma lesão na medula se correlacionam com o nível de independência para locomoção, higiene pessoal, controle do intestino, interação social medidos pela Escala de Independência Funcional (MIF), tipo de medicamento em uso e o subteste Linguagem da Escala MoCA. Diferentemente de estudos anteriores, não encontramos relação com uso de álcool e drogas ilícitas, níveis de lesão, diagnóstico etiológico ou tempo de lesão. Para os modelos de ansiedade, o principal preditor foram os sintomas de depressão, não sendo significante os aspectos relacionados com a LM. CONCLUSÃO: Foram encontradas que características da Escala MIF e aspectos cognitivos da Escala MoCA foram os principais preditores dos sintomas de depressão. Características da lesão e déficit motor não foram significantes. Estes achados potencialmente podem ser usados para orientar as práticas clínica visando identificar os pacientes com maior risco de apresentarem sintomas de depressão e ansiedadeINTRODUCTION: Spinal Cord Injury is a condition whose consequences can impact the physical, emotional, and social aspects of the life of the person who suffers it, as well as the dynamics of the social relationships that permeate it. High rates of psychological problems are observed after the onset of a disability, including depression and anxiety disorders, in addition to other forms of adaptive behavior and psychological distress. Thus, it is important to understand the predictors of depression and anxiety, as this knowledge can favor insights into the mechanisms of these mood changes in SCI. Still, the presence of depression and anxiety is a factor that influences the rehabilitation process, and this understanding is important for more efficient therapeutic planning, which can result in improved quality of life. OBJECTIVE: The main objective of this study was to identify the predictive and multivariate relationship between different sociodemographic and clinical variables of depression and anxiety symptoms in patients with spinal cord injury. METHOD: This was a cross-sectional study. Sociodemographic and clinical data were extracted from 556 SCI patients at the time of initial assessment, prior to intensive rehabilitation treatment at the Lucy Montoro Rehabilitation Institute. To identify the associated factors, we performed independent univariate and multivariate regression models using the Hospital Anxiety and Depression Scale (HADS). RESULTS: Our results showed that the symptoms of depression and anxiety present in patients affected by a spinal cord injury correlate with the level of independence for locomotion, personal hygiene, bowel control, social interaction measured by the Functional Independence Scale (FIM), type of medication in use and the Language subtest of the MoCA Scale. Unlike previous studies, we did not find a relationship with the use of alcohol and illicit drugs, injury levels, etiological diagnosis or duration of injury. For the anxiety models, the main predictor was depression symptoms, with SCI-related aspects not being significant. CONCLUSION: It was found that characteristics of the FIM Scale and cognitive aspects of the MoCA Scale were the main predictors of symptoms of depression. Characteristics of the injury and motor deficit were not statistically significant.
