Surgical treatment of comminuted mandible fracture involving simplification with miniplates: report of two cases

Bolsa MEC.Comminuted mandible fractures (CMF) have multiple fracture lines resulting in several bone fragments. Among the forms of treatment, open reduction and rigid internal fixation (FIR) seem to be the best option. This paper aims to report two cases of comminuted mandibular fracture treated by open reduction, simplification and rigid internal fixation. In both cases, the fracture was simplified with 1.5 mm mini plates and after the anatomy was restored, a 2.4 mm reconstruction plate and bicortical anchor screws were installed. This type of approach guarantees a stable occlusion, and the restoration of the mandibular anatomy. It is concluded that the open reduction and the FIR of comminuted fractures using the principle of simplification optimizes the results and reduces complications.Pesquisa sem auxílio de agências de fomentoAs fraturas cominutivas da mandíbula (FCM) apresentam múltiplas linhas de fraturas resultando em vários fragmentos ósseos. Entre as formas de tratamento a redução aberta e a fixação interna rígida (FIR) parecem ser a melhor opção. O presente trabalho tem como objetivo relatar dois casos de fratura mandibular cominutiva tratadas por redução aberta, simplificação e fixação interna rígida. Nos dois casos, a fratura foi simplificada com miniplacas de 1.5mm e após o restabelecimento da anatomia foi instalada uma placa de reconstrução de 2.4mm e parafusos de ancoragem bicortical. Esse tipo de abordagem garante uma oclusão estável, e o restabelecimento da anatomia mandibular.
