Study of C-Peptide and Glycoyslated Hemoglobinin Obese Individuals With and Without Type 2 Diabetes Mellitus in Rural Tertiary Care Hospital in Kancheepuram District

AIM OF THE STUDY: To estimate the levels of serum C-peptide and HbA1c in obese individuals with and without type 2 diabetes mellitus. To correlate the relationship between levels of C-peptide in obese individuals with and without type 2 diabetes mellitus and to correlate the relationship between levels of HbA1c, fasting and post-prandial blood glucose, lipid profile, in obese individuals with and without type 2 diabetes mellitus METHODS: An observational study has been carried out during the period of October 2018 to July 2020.Obese individuals diagnosed with Type 2 Diabetes Mellitus and Obese individuals without Type 2 Diabetes Mellitus with age group of 40 to 60 years were selected for the study. The total numbers of samples for the study were 30 cases and 30 controls attending the outpatient and Inpatient Department of General Medicine, Karpaga Vinayaga Institute of Medical Sciences & Research Centre. C-Peptide, Fasting and Post-Prandial Plasma Glucose,HbA1c, Renal function test and Lipid Profile was seen in both the groups. RESULTS: Along with insulin, C-peptide is produced in equal amounts and is the best measure for endogenous insulin in diabetic individuals. A strong positive correlation between BMI and C-peptide can be seen in both obese diabetic (p value =0.005) and obese non-diabetic (p value=0.000).This shows that as BMI increases, C-peptide also increases. Age,HbA1c and C-peptide can be used as predictive tools for predicting pre-diabetes in the non-diabetic population. CONCLUSION: Since, measurement of C-peptide is a better index of endogenous insulin production and pancreatic beta cell function than insulin measurements, this study suggests routine C-peptide testing along with HbA1c in high risk Non-Diabetic individuals for predicting the development of prediabetic state based on their age
