Analysis of Maternal and Fetal Outcome of Placenta Previa

1. In our study, the incidence of placenta previa contributed to 1.1% of the total deliveries. 2. In our study, the clinical presentation, risk factors, complications and outcome of 170 placenta previa cases were studied in detail. 3. In the present study the cases of placenta previa were highest in the maternal age group 25-29 years as 38% and least in the women in the age group less than 20 years as 3.5%. 4. In the present study incidence of placenta previa was highest in 60% the multiparous group. It was 30% in the primi group and 8.2% in grand multiparous group. 5. The incidence of placenta previa in prior caesarean section was 40%, prior abortion was 17% and myomectomy in 1.76% case. 6. Placenta associated with 28% of fetal malpresentations (breech 27%, transverse lie 1.7%) and 2% fetal anomalies. 7. In the present study 78% of cases required blood transfusion and PPH was noticed in 47% of cases, adherent placenta noticed in 15% cases, 29 cases of placenta previa required caesarean hysterectomy to control massive hemorrahage intraoperatively. 8. In the present study, perinatal morbidity was studied is 12% percentage of babies required resuscitation and NICU admission. 9. 32% of babies were low birth weight babies born with the birthweight of less than 2.5kg. 10. In the present study, the perinatal mortality rate was 4% and cause of mortality is mainly due to prematurity and birth Asphyxia. 11. The maternal mortality rate due to placenta previa was 1.18%. CONCLUSION: Placenta previa accounts for 1.1% of the total deliveries is associated with significant maternal morbidity and mortality and hence accurate diagnosis, proper and timely management is absolutely necessary. • The current study suggested there is association between advancing maternal age, gravidity parity previous abortion and cesarean sections as increased risk factors for placenta previa. • Efforts should also be made to promote birth spacing, limitation of family size, antenatal registration of all patients, and use of routine USG in all pregnant patients, early identification and referral of high risk mothers to tertiary care centers as it will help to reduce the adverse outcome of both mother and fetus. • Regional anesthesia may be safely administered in cases of placenta previa. Anticipation of the complications like PPH and conservative management may avoid serious consequences. • Since the incidence of preterm and low birth weight babies is high in cases of placenta previa delivery must be conducted in a tertiary care center with neonatal intensive care unit
