Effects of garlic on experimental myoglobinuric acute renal failure in rats

Miyoglobinürik akut böbrek yetmezliği iskelet kaslarının travmatik ya da travma dışı nedenlerle hasarlanması sonucu ortaya çıkan üremik bir sendromdur. Miyoglobinürik akut böbrek yetmezliği patogenezinde serbest radikallerin ve nitrik oksidin önemli rol oynadığı gösterilmiştir. Sarımsağın farklı organ ve dokularda antioksidan etki gösterdiği ve nitrik oksit düzeyini arttırdığı rapor edilmektedir. Sarımsağın deneysel miyoglobinürik akut böbrek yetmezliğinde etkilerini araştırmayı amaçladık. Çalışmamızda; Trakya Üniversitesi Deney Hayvanları Üretim ve Araştırma Laboratuvarı'nda yetiştirilen, 180-250 gram ağırlığında 40 adet Spraque-Dawley erkek sıçan kullanıldı. 1. ve 2. grup sıçanlar fizyolojik serum, diğer gruplar intramüsküler gliserol enjeksiyonundan 24 saat önce susuz bırakıldı. 1. ve 2. grup sıçanlara fizyolojik serum, 3. ve 4. gruplardaki sıçanlara %50'lik gliserol solüsyonundan 10 ml/kg'a göre bulunan toplam hacim eşit miktarlarda hafif eter anestezisi altında her iki arka bacak kaslarına enjekte edildi. 2. grup sıçanlara intramüsküler fizyolojik serum enjeksiyonundan 1 ve 24 saat sonra, 4. grup sıçanlara gliserol enjeksiyonundan 1 ve 24 saat sonra oral yolla 250 mg/kg dozunda sarımsak verildi. Gliserol enjeksiyonundan 48 saat sonra sıçanların rompun-ketamin anestezisi altında kan ve böbrekleri alındı. Böbrek dokusunda nitrik oksit, süperoksit dismutaz, katalaz, glutatyon peroksidaz enzim aktiviteleri, glutatyon, malondialdehit düzeyi; plazmada nitrik oksit, üre, kreatinin, sodyum ve potasyum düzeyleri; idrarda nitrik oksit, üre, kreatinin, sodyum, potasyum düzeyleri ile böbrekte histopatolojik değişiklikler incelendi. Çalışmamızın bulguları sonucunda 3. ve 4. grup arasında malondialdehit düzeyinde azalma ve doku nitrik oksit düzeyinde ve süperoksit dismutaz enzim aktivitesinde artma ile nekroz ve kast oranlarında azalma görüldü. Bu sonuçlar göz önüne alındığında sarımsağın miyoglobinürik akut böbrek yetmezliği üzerinde olumlu etkileri olduğu görüşündeyiz.Myoglobinuric acute renal failure is a uremic syndrome which develops due to traumatic or non-traumatic causes to skeletal muscle. Free oxygen radicals and nitric oxide play a crucial role in the pathogenesis of myoglobinuric acute renal failure. Garlic has been reported to have some antioxidant effects and increases nitric oxide levels in different tissues and organs. In this study, we aimed to investigate the effect of garlic on the experimental myoglobinuric acute renal failure.In our study, 40 male Spraque Dawley rats, 180-250 grams in weight, were used. The rats were randomly divided into 4 groups. After 24 hours water restriction in the diet, the rats in 1. groups and 2 received saline and those in 3. groups and 4, glycerol solution (50 %) were injected with at a ratio of 10 ml/kg in equal amounts in their hind limbs. One and 24 hours later, 1. groups and 3 received saline orally and 2. groups and 4 have taken 250 mg/kg of aqueous garlic. 48 hours after the glycerol injection, the blood samples and kidneys of the rats were taken under the anesthesia with xylazine-ketamin. The enzyme activities of superoxide dismutase (SOD), catalase and glutathione peroxidase and the level of nitric oxide, glutathione and malondialdehit were determined in the kidney tissue. The levels of urea, creatinine, nitric oxide, sodium and potassium were measured in the plasma samples and the levels of urea, creatinine, sodium, potassium and nitric oxide were examined in the urine specimens. Histopathological changes were also studied.The findings of study showed that when the 4. group compared to 3. group, the tissue MDA levels decreased, tissue nitric oxide levels and SOD enzyme activity significantly increased. In addition, necrosis and cast percentage significantly decreased. In the conclusion, we may suggest that garlic may have a protective effect on the experimental myoglobinuric ARF
