Treatment of Relapsed Acute Promyelocytic Leukemia: a Review of the Literature

Leucemia promielocítica aguda (LPA) é um subtipo de leucemia mielóide aguda (LMA) caracterizada, na maioria dos casos, pela translocação t(15;17) (q22;q21.1), que funde os genes codificantes da proteína PML (promyelocytic leucemia protein) e do RARA (retinoic acid receptor alpha). Previamente associada a um prognóstico muito reservado, a LPA é atualmente considerada uma doença altamente curável, o que pode ser atribuído à introdução dos agentes de terapia-alvo ácido all-trans retinóico (ATRA) e trióxido de arsénio (ATO) nos planos terapêuticos de primeira linha. A progressão do tratamento em direção a regimes de intensidade reduzida e baixa toxicidade permitiu uma grande melhoria dos outcomes e foi facilitada pela melhor compreensão da biologia celular e molecular da doença, assim como dos mecanismos de ação de alguns dos fármacos não-quimioterápicos utilizados no seu tratamento. No entanto, a recaída ainda ocorre em 10-15% dos doentes, maioritariamente entre aqueles com doença de alto-risco, surgindo como uma das principais causas de falência terapêutica na LPA. Apesar de a utilização de ATO ter mostrado grande eficácia também neste contexto, o tratamento ótimo a ser adotado em doentes em recaída não está ainda definido e a resistência a este agente, cujos mecanismos têm vindo a recentemente a ser estudados, deverá ser tida em consideração perante o processo de seleção do melhor regime de tratamento para doentes em recaída, assim como para o desenvolvimento de novos fármacos e abordagens alternativas no futuro.Acute promyelocytic leukemia (APL) is a subtype of acute myeloid leukemia (AML), in the majority of cases, characterized by the translocation t(15;17) (q22;q21.1), which fuses the genes encoding promyelocytic leukemia protein (PML) and retinoic acid receptor alpha (RARA). Previously associated with an extremely poor prognosis, APL is now considered a highly curable disease, following the introduction of target-therapy agents all-trans retinoic acid (ATRA) and arsenic trioxide (ATO) into frontline treatment regimens. This progression in treatment towards reduced intensity and low toxicity regimens has allowed for notably improved outcomes and was facilitated by the improved understanding of the cellular and molecular biology of the disease, as well as of the mechanisms of action of some of the non-chemotherapeutic agents used to treat it. However, relapse still occurs in 10-15% of APL patients, mostly within the high-risk disease subset, thus emerging as one of the main causes of treatment failure in APL. Although ATO has also been proven to be highly effective in this setting, the optimal treatment for patients with relapsed APL is yet to be defined, and newly discovered resistance to this agent should be considered while selecting the best treatment regimen for relapsed patients, as well as for future development of alternative agents and approaches
