The effect of sensorimotor interventions on feeding readiness and oral feeding success in preterm infants: A randomized controlled study

Bu çalışma, PAÜ Bilimsel Araştırma Projeleri Koordinasyon Birimi tarafından desteklenmiştir (Proje No: 2020SABE001).Araştırma, sensorimotor girişimlerin preterm bebeklerde beslenmeye hazır oluşluğa ve oral beslenme başarısına etkisini incelemek amacıyla yapılmıştır. Araştırma, Denizli Devlet Hastanesi’nin “Yenidoğan Yoğun Bakım Ünitesinde” yürütülmüştür. Veriler Haziran/2020-2021 tarihleri arasında toplanmıştır. Araştırmanın örneklemini (çalışma grubu 30, kontrol grubu 30) 60 preterm bebek oluşturmuştur. Araştırma verileri, Preterm Bebeği Tanıtıcı Bilgi Formu, Preterm Bebeklerde Oral Beslenmeye Hazır oluşluğu Değerlendirme Ölçeği (T-POFRAS), Altı Fazlı Besleme İlerleme Protokolü ile toplanmıştır (OBP). Çalışma grubunda T-POFRAS ölçek puanı 29’un altında olan bebeklere 10 gün boyunca taktil/kinestetik stimülasyonu ve besleyici olmayan emme olan kombine iki sensorimotor girişim uygulanmıştır. Kontrol grubuna ise yine T-POFRAS ölçek puanı 29’un altında olan bebeklere 10 gün boyunca sadece besleyici olmayan emme sensorimotor girişimi uygulanmıştır. Girişim sonrası her iki grupta da ölçek puanı 29 ve üzeri olan bebekler 12 günlük OBP’ye alınarak yarısı emzirme yarısı kap yöntemleri kullanılarak beslenmiştir. Beslenmeye hazır oluşluk verileri; sensorimotor girişim öncesi ve girişimden 10 gün sonra elde edilmiştir. Beslenme başarı düzeylerine ait veriler ise 12 günlük süre boyunca OBP’ye göre değerlendirilmiştir. Her iki grubunda tanımlayıcı klinik özellikleri homojen olup, müdahale öncesinde T-POFRAS puanları açısından istatistiksel olarak anlamlı bir farklılık bulunmamıştır (p>0,05). Çalışma ve kontrol gruplarına girişimden 10 gün sonra yapılan ölçümlerde T-POFRAS puanlarında istatistiksel olarak anlamlı farklılık tespit edilmiştir (p0,05). Elde edilen sonuçlar doğrultusunda kombine sensorimotor girişimlerinin, preterm bebeklerde oral beslenmeye hazır oluşlukta ve beslenme başarı düzeylerini artırmada etkili olduğu saptanmış olup preterm bebeklere bakım veren yenidoğan hemşirelerinin ve annelerin bu konu kapsamında bilgilendirilmesi önerilmektedir.The research was conducted to examine the effect of sensorimotor interventions on oral feeding readiness and oral feeding success in preterm infants. The research was carried out in the NICU of Denizli State Hospital. Data were collected between June/2020-2021. The sample of the study (30 in the study group, 30 in the control group) consisted of 60 preterm infants. Research data were collected with Preterm Baby Information Form, Preterm Oral Feeding Readiness Assessment Scale (T-POFRAS), Six-Phase Feeding Progression Protocol (SFPP). Two sensorimotor interventions combined with tactile/kinesthetic stimulation and non-nutritive sucking were applied for 10 days to infants with a T-POFRAS scale score below 29 in the intervention group. In the control group, only non-nutritive sucking sensorimotor intervention was applied for 10 days to preterm infants with a T-POFRAS scale score below 29. After the intervention, infants with a scale score of 29 and above in both groups were placed on a 12-day SFPP and fed half using breastfeeding and half cup methods. Feeding readiness data; were obtained before and 10 days after the sensorimotor intervention. Data on feeding success levels were evaluated according to SFPP during a 12-day period. The descriptive clinical features of the intervention and control groups were homogeneous, and no statistically significant difference was found in terms of T-POFRAS scores before the intervention (p>0.05). A statistically significant difference was found in the T-POFRAS scores of the intervention and control groups 10 days after the intervention (p0.05). In line with the results obtained, it has been determined that combined sensorimotor interventions are effective in increasing the readiness for oral feeding and feeding success levels in preterm infants, and it is recommended that neonatal nurses and mothers who care for preterm infants should be informed about this issue
