Comparison of different methods in evaluation of the subjective vertical in healthy controls and patients with menière's disease

In dieser Arbeit betrachteten wir zwei Parameter des Gleichgewichtssinns und ihre Bedeutung in der Diagnostik des endolymphatischen Hydrops. Die subjektive visuelle Vertikale (SVV) ist ein Parameter, der die Funktion des Utrikulus widerspiegelt und mit verschiedenen Methoden bestimmt werden kann. Die subjektive posturale Vertikale (SPV) ist ein propriozeptiver Parameter, der bei Rumpfexkursion bestimmt werden kann. In der Literatur finden sich zudem Angaben zu verkippter SVV bei gesunden Probanden bei Kopfneigung. In dieser klinisch prospektiven Studie verglichen wir mit verschiedenen Methoden die SVV bei gesunden Probanden und bei Patienten mit M. Menière in der chronischen Phase sowohl bei geradem Kopf als auch bei Kopfneigung sowie die SPV auf dem 3D-Rumpfexkursionsstuhl (3D-RES) mit und ohne Kopffixierung. Bei einer Stichprobe von gesunden Probanden ermittelten wir die SVV auf dem 3D-RES in ausgelenkter Position ohne Fixierung des Kopfes. Bei 39 gesunden Probanden (medianes Alter 25 Jahre, 30 weiblich, 9 männlich) und 26 Patienten mit M. Menière (medianes Alter 54,5 Jahre, 15 weiblich, 9 männlich) wurden die SVV bzw. die SPV mittels zwei verschiedenen SVV-Bucket-Tests (analog und digital), C-SVV®-Brille, 3D-Videookulographie und 3D-RES untersucht. 21 der gesunden Probanden wurden auf dem 3D-RES in Frontalebene ausgelenkt und haben die SVV erneut mittels C-SVV® bestimmt. Die statistische Auswertung erfolgte mit SPSS Version 25. Es gibt einen signifikanten Unterschied in der SVV-Wahrnehmung zwischen den gesunden Probanden und Patienten mit M. Menière. Der analoge Bucket-Test zeigt sich überlegen im Vergleich zur digitalen Variante. Es lässt sich eine Korrelation der SVV-Abweichung mit der Läsionsseite zeigen. Ein Einfluss von Betahistin auf das Untersuchungsergebnis wurde ausgeschlossen. Bei der SPV-Bestimmung gibt es zwischen den gesunden Probanden und den Patienten keine signifikanten Unterschiede, weder bei frei beweglichem Kopf noch bei Kopffixierung. Bei geneigtem Kopf gibt es sowohl bei Gesunden als auch bei Patienten eine signifikant stärkere Abweichung der SVV. Dieses Phänomen verstärkt sich, wenn die gesunden Probanden auf dem 3D-RES ausgelenkt werden. Eine verkippte SVV in Richtung der Läsion bei M. Menière kann ein Hinweis auf eine Utrikulus-Beteiligung auch in der chronischen Erkrankungsphase sein. Der analoge Bucket-Test überzeugte hierbei als präziseste Untersuchungsmethode. So kann dieser einfache, kostengünstige Test als zusätzliche diagnostische Methode bei M. Menière routiniert genutzt werden. Unsere Ergebnisse bestätigten, dass die SPV-Wahrnehmung hauptsächlich auf propriozeptivem Input beruht.The subjective visual vertical (SVV) is a parameter which evaluates the function of the utricle. In cases of menière’s disease (MD), the utricle can be affected by the endolymphatic hydrops so a deviation of the SVV can be expected. The subjective postural vertical (SPV) is mainly detected by proprioceptive input while the vestibular system is involved. In the literature, a description of a deviation of the SVV in healthy adults if the head is tilted can be found. In this study we compared the SVV in healthy adults and patients with MD while holding their head upright as well as tilted by using different methods as well as the SPV using a three-dimensional trunk-excursion chair (3D-TEC) with head moving freely and head fixed. In a sample of healthy adults we investigated the SVV while being tilted on the 3D-TEC. 39 healthy adults and 26 patients with at least probable MD were examined by using two different SVV-buckets (analog and digital), the C-SVV® goggles, the 3D-video oculography (3D-VOG) and 3D-TEC. 21 healthy adults set the SVV also while being tilted in roll axis on the 3D-TEC. Statistical procedures were performed on SPSS 25. There is a significant difference in the perception of SVV between the control group and patients with MD. The analog bucket test showed better results than the digital one. The SVV in patients is more likely to be tilted to the side of the lesion. An influence of betahistine in patients with MD was excluded. In the investigation of the SPV, there were no significant differences between the two groups, neither with the head moving freely nor with the head fixed. While tilting the head, the control group and patients showed a significant deviation of the SVV compared to the head being hold upright. This phenomenon is bigger when healthy adults perceived the SVV on the 3D-TEC. A tilted SVV to the side of the lesion in MD can be a hint to an affection of the utricle. The analog SVV-bucket showed the most sensitive results compared to the other examination tools. So this easy, cheap test can be used as an additional diagnostic method in MD. Our results confirmed the mainly proprioceptive input in the perception of the SPV
