Association between postoperative complications with intensive medical care and tumor-specific long-term mortality after debulking surgery in patients with ovarian cancer

Hintergrund: Da das fortgeschrittene Ovarialkarzinom insbesondere durch die Tumorausbreitung im gesamten Abdomen vom kleinen Becken bis zum Zwerchfell charakterisiert ist, ist das Tumor-Debulking ein aggressives Operationsverfahren und mit einem sehr hohen Risiko für postoperative Komplikationen verbunden. Sowohl postoperative Komplikationen als auch Überlebensdaten sind im Rahmen der Outcome-Analyse ein sehr wichtiges Instrument zur Qualitätsverbesserung. Das primäre Ziel dieser Arbeit war es nach einer multi-viszeralen Zytoreduktion aufgrund eines Ovarialkarzinoms, zum einen Assoziationen von patienten- und tumorspezifischen Charakteristika und anästhesiologischen sowie operativen Faktoren zu postoperativen Komplikationen zu untersuchen und zum anderen den langfristigen Effekt von postoperativen Komplikationen auf Mortalität und Rezidiv des Karzinoms zu analysieren. Methoden: Es handelt sich um eine retrospektive Analyse einer prospektiven Datenbank von Patientinnen, bei denen zwischen 2006 und 2014 eine vollständige zytoreduktive Operation mit makroskopischer Tumorfreiheit im Resektionsrand aufgrund eines primären Ovarialkarzinoms durchgeführt wurde. Eine postoperative Komplikation wurde als „jede Abweichung vom normalen beziehungsweise idealen postoperativen Verlauf“ nach der Clavien-Dindo-Klassifikation definiert. Es wurde der „Comprehensive Complication Index (CCI®)“ entsprechend der verwendeten Therapie und nach Clavien-Dindo-Klassifikation berechnet. Die Patientinnen wurden in zwei Gruppen eingeteilt: Niedrig- und Hoch-Index-Gruppe. Die Trennung entsprach dem CCI®-Wert von 42,4 beziehungsweise den Wert einer Komplikation Grad IVA der Clavien-Dindo Klassifikation. Patienten- und tumorspezifische Charakteristika und anästhesiologische sowie operative Faktoren wurden in Bezug auf postoperative Komplikationen untersucht. Die Konsequenzen von postoperativen Komplikationen auf den stationären postoperativen Verlauf und die Bedeutung für die Mortalität und Rezidive des Karzinoms wurden in der Gesamtpopulation und adjustiert durch Propensity-Score Matching (PSM) auf der Basis von patienten- und tumorspezifischen Risikofaktoren analysiert. Ergänzend wurde eine multivariate Cox-Regression durchgeführt. Ergebnisse: Insgesamt wurden 2114 postoperative Komplikationen in der Zeit ab dem Operationsende am Operationstag bis zur Krankenhausentlassung bei den analysierten 551 Patientinnen registriert. 9 von 10 Frauen haben während ihres Krankenhausaufenthaltes mindestens ein Ereignis, abweichend von idealem postoperativem Verlauf entwickelt, schwere Komplikationen (Clavien-Dindo Grad >IIIa) traten in 35,6% auf. Trotz des PSM hatten die Komplikationen einen signifikanten Einfluss [Hazard ratio (95 % Konfidenzintervall): 1,38 (1,01–1,89); p = 0,042] auf tumorspezifisches Langzeit-Überleben nach der Debulking-Operation für Ovarialkarzinom. Anästhesiologische Faktoren waren mit postoperativen Komplikationen in der direkten postoperativen Phase nach der Debulking-Operation für Ovarialkarzinom nicht assoziiert (P > 0,05). Schlussfolgerung: Die Ergebnisse dieser Arbeit zeigten, dass anästhesieassoziierte Faktoren keinen Einfluss auf die postoperativen Komplikationen hatten und dass ein Hoch-Index- oder vergleichbare Komplikationen für das Langzeit-Überleben nach einer Tumor-Debulking Operation bei Ovarialkarzinom relevant war. Diese Ergebnisse verdeutlichen die Notwendigkeit von Maßnahmen zur Komplikationsvorbeugung.Background: Since advanced ovarian cancer in particular is characterized by tumor spread throughout the entire abdomen from pelvis to diaphragm, tumor debulking is an aggressive surgical procedure and is associated with a very high risk for postoperative complications. Postoperative complications as well as survival data within an outcome analysis are a very important tool for quality improvement. The primary aim of this work was to analyze the association of patient- and cancer-related characteristics and anesthesia- and surgical factors to the development of postoperative complications and to examine the long-term effect of postoperative complications following multi-visceral abdominal cytoreduction due to ovarian cancer on mortality and cancer recurrence. Methods: This study is a retrospective analysis of a prospective database of patients who underwent complete cytoreductive surgery due to primary ovarian cancer with macroscopical tumor clearance in the resection area between 2006 and 2014. A postoperative complication was defined as "any deviation from normal or ideal postoperative course". The Comprehensive Complication Index (CCI®) was calculated according to the therapy used and the Clavien-Dindo classification. All patients were divided into two groups: lower- and higher-CCI groups. The split corresponds to the CCI® value of 42.4 respectively or the value of a complication grade IVA of the Clavien-Dindo classification. Correlations of patient- and tumor-specific characteristics and anesthesiological as well as surgical factors with postoperative complications were investigated. The impact of postoperative complications on the postoperative course in hospital and the relevance for mortality and cancer recurrence were analyzed in the total population and adjusted by means of Propensity Score Matching (PSM) for patient- and tumor-specific risk factors. Additionally, multivariate Cox regression was performed. Results: A total of 2114 postoperative complications were registered from the end of surgery up to hospital discharge in the analyzed 551 patients. 9 out of 10 women developed at least one event during their hospital stay deviating from ideal postoperative course. Major complications (Clavien-Dindo grade >IIIa) occurred in 35.6%. Despite PSM the complications had a significant impact [Hazard ratio (95 % confidence interval): 1,38 (1,01–1,89); p = 0,042] on tumor-specific long-term survival after debulking surgery for ovarian cancer. Anesthesiologic factors were not associated with postoperative complications in the immediate postoperative phase after debulking surgery for ovarian cancer (P > 0.05). Conclusion: The results of this study show no influence of anesthesia-associated factors on postoperative complications and that a high CCI® or comparable complications are relevant for long-term survival after tumor debulking surgery for ovarian cancer.
