Coiffage pulpaire direct : indications, protocole et perspectives

Nowadays, during his practice, the dentist has for essential preoccupation the conservation of the pulp vitality. One of the procedures of this conservation is the direct pulp capping: this is a treatment which consists, in case of pulp exposure and through a biomaterial, to maintain the pulp vitality of a tooth. Calcium hydroxide is the first material to be used and is considered to be the "gold standard" for pulp capping. However, in recent years, materials based on calcium silicate have appeared and are excellent alternatives to calcium hydroxide: we speak of Mineral Trioxide Aggregate (MTA), Biodentine™ and Theracal. This work presents, in a first part, the different indications and the local and general contraindications to the technique of direct pulp capping. The second part presents the different biomaterials available, and their physicochemical and biological properties. The clinical operating steps as well as the follow-up and prognosis are then treated. The third and last part proposes a discussion around this clinical procedure and its perspectives.De nos jours, lors de sa pratique, le chirurgien-dentiste a pour préoccupation essentielle la conservation de la vitalité pulpaire. Une des procédures de cette conservation est le coiffage pulpaire direct : c’est un traitement qui consiste, en cas d’exposition pulpaire et par l’intermédiaire d’un biomatériau, à maintenir la vitalité pulpaire d’une dent. L’hydroxyde de calcium est le premier matériau à être utilisé et considéré comme le "gold standard" en matière de coiffage pulpaire. Cependant, depuis quelques années, des matériaux à base de silicate de calcium ont fait leur apparition et sont d’excellentes alternatives à l’hydroxyde de calcium : on parle du Mineral Trioxyde Aggregate (MTA), de Biodentine™ et de Theracal. Ce travail présente, dans une première partie, les différentes indications et contre-indications locales et générales à la technique du coiffage pulpaire direct. La deuxième partie présente les différents biomatériaux utilisables, ainsi que leurs propriétés physico-chimiques et biologiques. Les étapes opératoires cliniques ainsi que le suivi et les pronostics y sont ensuite traités.
