Comparison of efficiency of manual lymphatic drainage and low level laser treatment in impacted lower molar surgery

Çalışmamızın amacı gömülü alt üçüncü molar diş (GAÜMD) cerrahisinde manuel lenfatik drenaj (MLD) ve düşük enerjili lazer tedavisinin (DELT) postoperatif ağrı, ödem, trismus ve yaşam kalitesi üzerindeki etkinliğini karşılaştırmaktır. Çalışmaya yaş ortalaması 25.7±5.9 olan 36 hasta dahil edildi. Hastalar MLD, DELT ve kontrol grubu olmak üzere 3 gruba randomize edildi. Bireylere bulundukları gruba göre rutin tedavinin yanında postoperatif 24., 48. ve 72. saatlerde 3 gün boyunca günde 1 defa MLD ve DELT uygulandı. Kontrol grubu yalnızca buz uygulaması ve rutin ilaç tedavisi aldı. Hastalara ağrı (Görsel Analog Skala-VAS), ödem (kraniometrik ölçüm), trismus (maksimum interinsizal uzaklık ölçümü) ve yaşam kalitesi (Ağız Sağlığı Etki Profili-OHİP-14) değerlendirmeleri yapıldı. Değerlendirmeler operasyondan önce, operasyon sonrası 24., 48. ve 72. saatler ve postoperatif 7.gün gerçekleştirildi. Veri analizi için SPSS 22.0 paket programı kullanıldı. İstatistiksel analizler sonucunda, her iki tedavi grubunun 4. günden 7. güne kadar olan ortalama ağrı düzeyi ve tedavi süresince ilave ağrı kesici kullanımı kontrol grubuna kıyasla anlamlı olarak daha düşük bulundu. Tedavi grupları arasında preoperatif döneme göre postoperatif 7.gün ortalama ödem artışı ve ağız açıklığı değeri anlamlı farklılık göstermezken kontrol grubunda anlamlı olarak daha yüksekti. Grupların yaşam kalitesi değerlendirmesinde tedavi gruplarının lehine anlamlı farklılık saptandı. Bu çalışmada GAÜMD cerrahisi sonrası MLD ve DELT'nin ağrı ve ödemi azaltma, ağız açıklığı ve yaşam kalitesini artırma açısından klinikte etkin şekilde kullanılabilecek tedavi yöntemleri olduğu görüşüne varıldı. Ayrıca çalışmamızın gelecekte oral ve maksillofasiyal cerrahinin yanı sıra ortognatik cerrahi gibi alanlarda MLD tedavisinin uygulanacağı çalışmalara bir ön zemin oluşturacağı düşünülmektedir.The aim of this study was to compare the efficacy of manual lymphatic drainage (MLD) and low level laser therapy (LLLT) on postoperative pain, edema, trismus and quality of life in impated lower third molar tooth (ILTMS) surgery. The study included 36 patients with a mean age of 25.7 ± 5.9 years. Patients were randomized into 3 groups: MLD, DELT and control groups. In addition to routine treatment, MLD and LLLT were administered once a day for 3 days at the 24th, 48th and 72th hours postoperatively. The control group received only ice and routine medication. Pain (Visual Analogue Scale-VAS), edema (craniometric measurement), trismus (maximum interinsisal distance measurement) and quality of life (Oral Health Impact Profile-OHIP-14) were evaluated. Evaluations were performed before the operation. 24th, 48th and 72th hours postoperatively and on the 7th postoperative day. SPSS 22.0 software was used for data analysis. As a result of the statistical analyzes, the mean pain level between the 4th and 7th days of the two treatment groups and the use of additional pain medication during the treatment were significantly lower than the control group. The mean edema increase and mouth opening values did not differ significantly between the treatment groups in the postoperative 7th day compared to the preoperative period, but it was significantly higher in the control group. There was a significant difference in the quality of life evaluation of the groups in favor of the treatment groups. In this study, it was concluded that MLD and LLLT after ILTMS surgery can be used effectively in the clinic to reduce pain and edema, improve mouth opening and quality of life. In addition, it is thought that our study will provide a basis for future studies on MLD treatment in areas such as oral and maxillofacial surgery as well as orthognathic surgery
