Use of renal near infrared spectroscopy in determining cardiopulmonary bypass-related acute kidney damage in heart surgery

Kardiyopulmoner bypass sonrası akut böbrek hasarı (ABH) erken ve geç dönemde hasta üzerinde ciddi etkiler oluşturmaktadır (1). Bu nedenle ABH'nin erken dönemde tespiti faydalı olacaktır. Bu çalışmadaki amacımız kalp cerrahisi uygulanan infant hastalara preoperatif dönemden başlayarak yoğun bakım sürecinin 24. saatine kadar olan süreçte renal near infrared spectroscopy (rNIRS) takibi yaparak, ABH'nin erken dönemde tespitine faydasını araştırmaktır. Çalışma prospektif tek merkezli olup, hastanemizde açık kalp ameliyatı yapılan 1 yaş altı 62 pediatrik hasta dahil edilmiştir. Hastalardan; preoperatif, operatif ve postoperatif olmak üzere kan gazı değerleri (ph, SaO2, BE, hct, laktat), BUN, kreatinin, glomerüler filtrasyon hızı (GFR), idrar çıkışı, drenaj miktarları, preoperatif dönemden postoperatif 24. saat aralığındaki serebral NIRS (cNIRS) ve rNIRS takipleri kaydedildi. Postoperatif 6., 24., 48. saatlerde pediatrik RIFLE ; R (risk), I (injury: hasar), F (failure: yetersizlik), L (loss: kayıp) ve E (end stage kidney injury: son dönem böbrek hasarı) sınıflandırması, kreatinin klirens değerleri hesaplandı. ABH tanısı için postoperatif 24. saat pRIFLE sınıflamaları ayrı ayrı hesaplandı. İstatiksel veriler sonucunda ABH gelişen 25 ve ABH gelişmeyen 37 hasta arasında preoperatif ortalama arter basıncı (OAB) ve PaCO2 değerinde, postoperatif 2.saat ve 24.saat aralığında OAB 50 mmHg altı değerlerinde, serum kreatinin 24.saat ve kreatinin klirens 24.saat değerlerinde anlamlı farklılık bulunmuştur (p0.05). Sonuç olarak yaptığımız bu çalışmada rNIRS kullanımının kardiyopulmoner bypass (KPB) sonrası ABH erken tanısı için belirleyici bir rolü bulunamamıştır.Acute kidney injury (AKI) that develops after cardiopulmonary bypass has serious effects on patients in the early and late periods (1). Therefore, it will be beneficial to detect AKI in the early period. Our aim in this study is to investigate the benefit of early detection to AKI by monitoring renal near infrared spectroscopy (rNIRS) in the period from the preoperative period until the 24th hour of the intensive care process on infant patients who had cardiac surgery. The study was prospective and single-center and included 62 pediatric patients under 1 year old who underwent open heart surgery in our hospital. Preoperative, operative and postoperative blood gas values (ph, SaO2, BE, hct, lactate), BUN, creatinine, glomerular filtration rate (GFR), urine output and postoperative drainage values were measured; cerebral NIRS (cNIRS) and rNIRS follow-ups were recorded from the preoperative period to the 24th postoperative hour. Pediatric RIFLE (R (risk), I (injury), F (failure), L (loss) and E (end stage kidney injury)) classification and creatinine clearance values were calculated at postoperative 6th, 24th, 48th hours. For the diagnosis of AKI, pRIFLE parameters were calculated individually at the postoperative 24th hour. As a result of statistical data, a significant difference was found between 25 patients who developed AKI and 37 patients without AKI, in preoperative mean arterial pressure (MAP) and PaCO2 values, between 2 and 24 hours postoperative MAP values measured at 0.05). As a result, in this study, the use of rNIRS could not have a decisive role in the early diagnosis of AKI after cardiopulmonary bypass (CPB)
