Schema therapy for borderline personality disorder

Borderline personality disorder (BPD) is a serious and highly prevalent mental disorder among outpatient and inpatient individuals treated in health clinics. Psychotherapy is the treatment of choice and currently there are different psychotherapies for the treatment of BPD with studies that support their effectiveness. There is however still a great need to increase the evidence. A literature review conducted by Sempertegui, Karreman, Arntz and Bekker (2013) found evidence that supports schema therapy (ST) as a promising and comprehensive treatment approach for BPD. However, the number of efficacy studies were small. Furthermore, only one of the studies included a theory-specific outcome. The aim of this thesis was to provide a summarized and updated review of studies regarding the effectiveness of ST for the treatment of BPD. A literature search was conducted with the search words (schem*) OR (EMS) AND (”borderline personality disorder”). The search engines used were PsycInfo, Web of Science (WoS) and Medline. A total of six studies were included. All of the included studies found ST to be effective in the treatment of BPD. The effect sizes were large. Three of the studies included theory-specific outcomes, which have shown declines in early maladaptive schemas (EMS) and dysfunctional modes and increases in functional modes. Evidence also suggests that ST is cost-efficient. Further research should be executed in order to replicate the results and increase the evidence of ST as an efficient treatment of BPD. Psykoterapi anbefales som primær behandling for BPF hvor enkelte psykoterapier har vist behandlingseffekt. Det er imidlertid behov for å øke evidensgrunnlaget. En litteraturgjennomgang av Sempertegui, Karreman, Arntz og Bekker (2013) fant evidens for skjematerapi (ST) som en lovende og helhetlig behandlingstilnærming for BPF. Et lite antall effektstudier, hvor kun en av studiene inkluderte et teorispesifikt utfallsmål, begrenser generaliserbarheten av disse funnene. Hensikten med denne litteraturgjennomgangen var dermed å gi et samlet og oppdatert bilde av hva forskning viser av behandlingseffekt for ST for BPF. Litteraturgjennomgangen har blitt utført ved et systematisk litteratursøk, med søkeordene (schem*) OR (EMS) AND (”borderline personality disorder”). Søket har blitt utført i databasene PsycInfo, Web of Science (WoS) og Medline. Seks empiriske studier er inkludert i oppgaven, som alle har vist reduksjon i BPF-symptomer med store effekter. Tre av studiene inkluderte teorispesifikke utfallsmål, som har vist nedgang i tidlig maladaptive skjema (TMS) og dysfunksjonelle moduser og økning i funksjonelle moduser. Det er videre evidens for at behandlingen er kostnadseffektiv. Med henhold til viktige begrensninger i metode og design som er adressert i oppgaven bør funnene imidlertid tolkes med forsiktighet. Mer forskning er nødvendig for å replikere funnene og øke evidensgrunnlaget for ST for BPF.
