Strategies to increase the effectiveness of wound healing therapy with mesenchymal stem cells in diabetic patients

Przeszczep z użyciem mezenchymalnych komórek macierzystych (MSC) podawanych w obręb rany wydaje się obiecującym sposobem leczenia trudno gojących się ran u chorych na cukrzycę. Nie ma obecnie szczegółowych wytycznych, które w sposób wystandaryzowany określałyby metodykę zastosowania takiej formy terapii. Najnowsze wyniki badań wskazują jednak na wielokierunkowy efekt działania MSC (poprawa warunków ukrwienia, poprawa ziarninowania tkanek w obrębie rany, wytworzenie powierzchownych warstw skóry), jak i różnice w efektywności pomiędzy przeszczepieniem komórek pochodzących od osoby zdrowej (przeszczep allogeniczny), w porównaniu z przeszczepieniem komórek gospodarza (przeszczep autologiczny). Modyfikując w różny sposób terapię z użyciem MSC, można uzyskać szybszy efekt gojenia się ran, a sama terapia może stanowić przełom w leczeniu przewlekłych ran u chorych na cukrzycę. W artykule zaprezentowano przegląd najnowszych doniesień w zakresie zastosowania MSC w leczeniu trudno gojących się ran w przebiegu cukrzycy.In the treatment of hard-to-heal wounds in diabetes patients, transplantation using mesenchymal stem cells (MSCs) administered into the wound seems to be a promising treatment method. We still do not have algorithms precisely defining the proper usage of this form of therapy. Nevertheless, the latest research results indicate multidirectional therapy effects (improvement of blood supply conditions, tissue granulation within the wound, creation of superficial layers of the skin), as well as differences in the effectiveness between transplantation of cells obtained from a healthy person (allogeneic transplantation) and transplantation of cells obtained from host (autologous transplantation). By modifying the therapy at the above-mentioned ways, not only can we achieve faster wound healing, but also the therapy itself can be a breakthrough in the treatment of chronic wounds in diabetic patients.
