This may lead to different challenges to implementation of the Patient Aligned Care Team (PACT) model, VHA’s Patient-Centered Medical Home (PCMH) model. OBJECTIVE: To determine whether PACT implementation at VHA primary care clinics varied by rural or urban clinic classification.
