Le syndrome de Poland de decouverte tardive au Togo, cas clinique et revue de la litterature

Introduction: Le Syndrome de Poland, une pathologie congénitale associe à des degrés divers des anomalies thoraciques pariétales, mammaires, et des malformations distales du membre supérieur ipsilatéral.Objectifs: Rapporter un cas de syndrome de Poland découvert à l’âge adulte au Togo, et faire une revue de la littérature.Observation: Une étudiante de 27 ans sans antécédent particulier a consulté pour une malformation thoracique droite apparue depuis l’enfance. La malformation était caractérisée par une absence de son sein droit et une malformation de la main droite. La glande mammaire droite était totalement absente (amastie), le sein gauche était présent et normal. Les membres supérieurs étaient de même longueur entre les épaules et les poignets. La main droite présentait une symbrachydactylie. L’échographie mammaire et le scanner thoracique avaient confirmé l’absence totale de la glande mammaire du côté droit et une agénésie du muscle grand pectoral. A l’échographie Doppler vasculaire les artères sous-clavières étaient de même calibre et étaient symétriques. La patiente a refusé la chirurgie de reconstruction mammaire, elle a préféré porter des conformateurs externes.Conclusion: Le syndrome de Poland est une malformation congénitale dont le diagnostic est clinique. There was a total right amastia. The left breast was normal. The upper limbs were of same length between shoulder and wrist. There was a symbrachydactyly at the right hand. An echography and a chest scannography confirmed the absence of the right breast and an agenesis of the pectoralis major. A Doppler Echography showed subclavian arteries and its branches of symmetric size with no occlusion. The student refused the breast reconstruction. She preferred using external conformators.Conclusion: Poland syndrome is a congenital pathology that diagnosis rely on clinical examination.
