Prevalence of cardiovascular risk factors and non alcoholic fatty liver disease in military personnel and their correlation before and after therapeutic intervention

Background: Non-alcoholic fatty liver disease (NAFLD) and its severe form non-alcoholic steatohepatitis (NASH) are major health problems worldwide. They are the hepatic manifestation of the metabolic syndrome and are connected to increased cardiovascular risk as well as the development of cirrhosis and hepatocellular cancer. Genetics may play a role in the pathogenesis of NAFLD/NASH. The aim of the present study is to show that therapeutic intervention and modification of the phenotypic expression of genetic factors with statins has a beneficial effect on the progress of the hepatic disease.Aim: To investigate the prevalence of NAFLD/NASH in 5,400 military personnel and evaluate the effect of treatment with three statins (atorvastatin, rosuvastatin, pitavastatin) on NAFLD/NASH using two non-invasive scores [NAFLD Activity Score (NAS); reference range 0-8; Fibrosis-4 score (FIB-4); reference range 1.45-3.25]. Methods: During the mandatory annual medical check-up, all military personnel from combat infantry units underwent a clinical and laboratory evaluation. The NAS and FIB-4 scores were calculated based on these values. Participants with NAFLD/NASH were randomised to four treatment groups (n=151each): diet-exercise, atorvastatin, rosuvastatin or pitavastatin for 1 year, when another routine evaluation was carried out.Results: From all the participants, 613 had NAFLD/NASH (prevalence 11.3 vs 39.8% in the general population, p&lt;0.0001); 604 consented to participate in the study. After a year of treatment, the diet-exercise group showed no significant changes in both scores (NAS 4.98 baseline vs 5.62, p=0.07; FIB-43.42 vs 3.52, p=0.7). For the atorvastatin group, both scores were reduced (NAS 4.97 vs 1.95, p&lt;0.001, FIB-4 3.56 vs 0.83, p&lt;0.001), for rosuvastatin (NAS 5.55 vs 0.83, p&lt;0.001, FIB-4 3.61 vs 0.79, p&lt;0.001), and for pitavastatin (NAS 4.89 vs 0.85, p&lt;0.001, FIB-4 3.59 vs 0.87, p&lt;0.001).Conclusions: Non-alcoholic fatty liver disease (NFLAD) and non-alcoholic steatohepatitis (NASH) are related to increased morbidity and mortality due to hepatic cirrhosis or/and hepatocellular cancer. Proper therapeutic treatment of NAFLD/NASH may have satisfactory results in the prevention of the above-mentioned diseases. Atorvastatin, rosuvastatin and pitavastatin have a beneficial and safe effect in NAFLD/NASH patients as recorded by the improvement in the NAS (representing NAFLD activity) and FIB-4 (representing liver fibrosis) scores. Since both the participants with NAFLD/NASH as well as those without NAFLD/NASH had similar characteristics, genetics may play a significant role both in NAFLD/NASH pathogenesis and its treatment with a statin. Statins may reduce the risk of progression to serious disease by modifying the phenotypic expression of these genetic factors.Υπόθεση : Η μη αλκοολική λιπώδης νόσος του ήπατος (ΜΑΛΝΗ) και η σοβαρή της μορφή, η μη αλκοολική στεατοηπατίτιδα είναι σημαντικά προβλήματα υγείας παγκοσμίως. Αποτελούν την ηπατική έκφραση του μεταβολικού συνδρόμου και σχετίζονται τόσο με αυξημένο καρδιαγγειακό κίνδυνο όσο και με την πιθανότητα ανάπτυξης κίρρωσης/ηπατοκυτταρικού καρκίνου. Η γενετική μπορεί να παίζει ρόλο στην παθογένεση της ΜΑΛΝΗ/ΜΑΣΗ. Ο στόχος της παρούσας μελέτης είναι να καταδείξει ότι η θεραπευτική παρέμβαση και τροποποίηση της φαινοτυπικής εκδήλωσης των γενετικών παραγόντων παθογένεσης της ΜΑΛΝΗ/ΜΑΣΗ με στατίνες πιθανόν να έχει ευεργετική επίδραση στην εξέλιξη της ηπατικής νόσου. Στόχος: Να διερευνήσει τον επιπολασμό της ΜΑΛΝΗ/ΜΑΣΗ σε 5.400 στρατιωτικούς και να αξιολογήσει την επίδραση της θεραπείας με τρεις στατίνες (ατορβαστατίνη, ροσουβαστατίνη, πιταβαστατίνη ) στη ΜΑΛΝΗ/ΜΑΣΗ χρησιμοποιώντας δύο μη επεμβατικές βαθμολογίες [ΜΑΛΝΗ Activity Score (NAS) εύρος αναφοράς 0-8˙ Βαθμός ίνωσης-4 ( FIB-4)˙ εύρος αναφοράς 1,45 έως 3,25]. Μέθοδοι : Κατά τη διάρκεια του υποχρεωτικού ετήσιου υγειονομικού ελέγχου, όλο το στρατιωτικό προσωπικό από μονάδες μάχης του στρατού ξηράς υποβλήθηκε σε κλινική και εργαστηριακή αξιολόγηση. Οι βαθμολογίες NAS και FIB-4 υπολογίστηκαν με βάση αυτές τις τιμές.
