Pour une approche des fonctionnements psychiques sous l'angle de la survie psychique

International audienceAbstractObjectivesFor those whose psychic economy aims towards survival rather than towards the quest for pleasure, the overwhelming by organic sensations, whether without affect or hallucinated, leads to symptom profiles such as hypochondria and depersonalization, in a dual process that is markedly paradoxical. While hypochondria and depersonalization are a threat to the continuity of being, they also protect it. This dual process leads us to the notion of psychic survival and the aim here is to try to understand how far this can enable us to apprehend borderline functioning and its treatment, using an analysis of transference and countertransference.MethodIt was clinical work with adolescents that made me sensitive to this paradoxical outcome; but it was the borderline patients encountered in my private practice – adults and young adults – that led me to the idea that these strangely unsettling symptom profiles could also reflect a form of psychic survival. The transference-countertransference movements within the analytical process, as well as the reflective abilities of these subjects “at the borders of insanity”, then led me to consider the battle they were waging against collapse and their defensive strategies to contain the return of “psychic death equivalents” as being part of a “crisis of the erotic body”, which should be understood n relation to a poorly metabolized body.ResultsFor “borderline” patients, the Ego is protected from its dissolution but never spared this perspective. This tension affords wide scope for approaching borderline functioning from the angle of psychic survival, which could hardly be more distinct than in the case of hypochondria and depersonalization. This approach leads us in particular to consider that the overinvestment in bodily limits enables these subjects to “recover” their bodies and the time-line when the psychosomatic release is at its peak. Indeed, for them, while the disappearance of the object results in the “belief” in their own disappearance, it has nonetheless left a mark in their psychic economy and impacted the energy flows. This, therefore, invites us to consider that there may be an “economy of survival” inherent in borderline functioning.DiscussionFor these subjects, the mere self-observation of the “disturbing stranger” within themselves slows the progressive loss of the feeling of continuity, the refocusing on the sensations of the body and the over-investment of the self-observing part of the Ego designed to restrain the psychosomatic unbinding that heralds risk-prone excesses. Thus, the symptomatic forms of self-observation enable the “I” to continue to invest in itself and to represent itself, and finally to sustain reflective function.ConclusionThe approach to borderline functioning in perspective of psychic survival, therefore, leads us to suggest that these subjects’ “insanity” finds its limits at the boundaries of the body, while never ceasing to return within it. It also suggests the need to explore the issues of transference and countertransference in relation to the questions of self-vigilance and hypervigilance as providing a privileged access to the “dead zones” of the psyche.RésuméObjectifsPour les sujets dont l’économie psychique tend davantage vers la survie que vers la recherche de plaisir, l’envahissement par des sensations organiques sans affect ou hallucinées donne lieu à des formations symptomatiques, telles l’hypocondrie et la dépersonnalisation, qui participent d’un double mouvement empreint de paradoxalité. Car si l’hypocondrie et la dépersonnalisation menacent la continuité d’exister, elles y font également rempart. Ce double mouvement nous engage ainsi sur la voie de la survie psychique, dont nous chercherons ici à saisir dans quelle mesure elle nous permet d’appréhender les fonctionnements limites et leur traitement à partir de l’analyse des mouvements transféro/contre-transférentiels.MéthodeC’est tout d’abord la clinique des adolescents qui m’a rendue sensible à ce destin paradoxal, mais ce sont des patients « limites » rencontrés dans un cadre privé – adultes et jeunes adultes – qui m’ont amenée à l’idée que ces formations symptomatiques « étrangement inquiétantes » pouvaient également relever de la survie psychique. Les mouvements transféro/contre-transférentiels dans l’espace analytique et la capacité réflexive de ces sujets « au bord de la folie » m’ont alors fait envisager la lutte qu’ils menaient contre l’effondrement et leurs stratégies défensives pour endiguer le retour d’« équivalents de mort psychique », comme relevant d’une « crise du corps érotique », à entendre du côté d’un corps mal métabolisé psychiquement.RésultatsChez les patients « limites », le Moi est protégé de sa dissolution, mais il n’est jamais épargné de cette perspective, tension dans laquelle réside toute la richesse d’une approche des fonctionnements limites sous l’angle de la survie psychique, on ne peut plus nette à l’épreuve de l’hypocondrie et de la dépersonnalisation. Cette approche nous conduit notamment à envisager que le surinvestissement des limites corporelles permet à ces sujets de reprendre corps et de reprendre la trame du temps, lorsque la déliaison somato-psychique est à son comble. Car pour eux, si la disparition de l’objet a bien signifié la « croyance » en leur propre disparition, elle a néanmoins laissé une trace dans l’économie psychique et eu des effets sur la circulation du flux énergétique, qui nous invitent à réfléchir sur une économie de survie dans les fonctionnements limites.DiscussionPour ces sujets, l’auto-observation de l’« étranger inquiétant » en soi vient freiner la perte progressive du sentiment d’exister, le recentrage sur les sensations du corps et le surinvestissement de la partie auto-observatrice du Moi venant faire « limite » à la déliaison somato-psychique annonciatrice de « franchissements à risques ». Dans ces conditions, les formes symptomatiques de l’auto-observation permettent au « Je » de continuer à s’auto-investir et à s’autoreprésenter et finalement à s’assurer d’une fonction réflexive.ConclusionL’approche des fonctionnements limites sous l’angle de la survie psychique nous amène ainsi à proposer l’idée que leur « folie » trouve une limite aux frontières du corps tout en ne cessant pas de faire retour en son sein et nous fait envisager l’étude des enjeux transféro/contre-transférentiels autour des questions de l’auto- et de l’hyper-vigilance comme une voie d’accès privilégiée vers les « zones mortes » de la psyché
