Comparative study between the Goldmann applanation tonometer and non-contact tonometer in patients with primary open angle glaucoma

Mestrado em Gestão e Avaliação de Tecnologias em SaúdeObjetivo: Avaliar se o Tonómetro de Não Contacto (TNC) é uma alternativa apropriada ao Tonómetro de Aplanação de Goldmann (TAG) para medir a Pressão Intraocular (PIO). Métodos: A Pressão intraocular (PIO) foi medida através do TNC Topcon CT-80A e pelo TAG AT 900 Haag-Streit, em 106 olhos de 53 pacientes normotensionais e em 82 olhos de 41 pacientes com Glaucoma Primário de Ângulo Aberto (GPAA). Resultados: Os grupos são homogéneos relativamente à idade, ao género, à paquimetria, à queratometria. O TNC dá valores médios superiores aos do TAG, em ambos os olhos e em ambos os grupos. No grupo controlo para ambos os olhos e no grupo em estudo no OE existem diferenças significativas entre a medida dos dois tonómetros (p<0.005). Através da análise de Bland e Altman verificou-se que no grupo controlo no OD, 81.145% da diferença das medidas entre ambos os tonómetros estavam entre ± 3mmHg, no OE 86.792% estavam entre ± 3mmHg. No grupo em estudo no OD, 80.487% das diferenças das medidas medições estiveram entre os ± 3mmHg e no OE 75.609% estavam entre ± 3mmHg. Observou-se uma correlação linear fraca positiva não significativa entre a PIO e a ECC e uma correlação fraca negativa não significativa entre a curvatura da córnea e a PIO. Conclusões: Em indivíduos normotensionais é recomendável a comprovação através do TAG em valores elevados de ECC (ECC>580 μm). Em doentes com diagnóstico de GPAA, PIO obtidas pelo TNC com valores superiores a 16mmHg necessitam de comprovação pelo TAG. Tendo em consideração todas estas situações o TNC pode ser utilizado como método clínico objetivo para medir a PIO.ABSTRACT - Purpose: To evaluate if non-contact tonometer (NCT) is suitable as alternative to Goldmann Applanation Tonometer (GAT) for measuring intraocular pressure (IOP). Methods: PIO was measured with an NCT Topcon CT-80A and GAT AT 900 Haag-Streit in 106 eyes of 53 patients with normal IOP and 82 eyes of 41 patients with Primary Open Angle Glaucoma (POAA). Results: The groups are homogeneous with respect to age, gender, paquimetry and keratometry. The NCT gives higher readings than GAT in both eyes and both groups. In control group for both eyes and in study group for left eye there was a statistically significant difference between the measurements using both tonometers (p<0.005). Using the method of Bland and Altman was found that in control group in right eye, 81.145% of differences of measures between both tonometers were among ±3mmHg and in left eye 86.792% were between ±3mmHg. In study group in right eye 80.487% of differences of measures were among ±3mmHg and in left eye 75.609% were between ±3mmHg. There was observed a linear weak positive not significant correlation between IOP and Central Corneal Thickness (CCT) and linear weak negative not significant correlation between IOP and Corneal Curvature. Conclusion: In patients with normal IOP is recommended the confirmation through the GAT in high values of CCT (CCT>580 μm). In patients with POAA, IOP obtained by NCT with values higher than 16mmHg need attestation by GAT. Taking into consideration all these situations the NCT con be used as objective clinical method to measure the IOP
