Hôpital, Territoire, Santé : l'émergence d'un contrôle de gestion médicalisé ?

A lot of research has dealt with the emergence of an hospital management control function since the elaboration of the activity-based payment model. However, management control seems to suffer from a certain number of issues such as the proliferation of budgetary tools without link with actors' logics and without impact on the organizational behavior. This view of management control seems to us reducing since the creation of Health Care Delivery Head Office and the new governance of health system linked with the creation of Regional Agencies of Health. It shows the will asserted having a global approach of health care delivery joining as well ambulatory care and hospital. Studies in management outline a management control from resource allocation to performance management linked with strategy. This PhD thesis analyzes the deficiencies of traditional tools stemming from hospital cost accounting in the context of a destabilization of the object "hospital". It bases on the hypothesis according to which emergence of a medical management control is similar to an eco-design approach which connects the construction of the instrumentation with that of the level of the organization. The proposition is led around three grounds on the model of intervention research classified according to two variables: the degree of organizational innovation on the one hand, characterized by new coordination methods, and the distance with regard to the traditional instrumentation of management control on the other hand. However, even if we adopt a more ambitious approach linked with organizational dynamics which is more and more complicated because of the opening outside of the hospital, we don't claim to question the development and the use of the cost accounting system in hospital. The tools which ensue from it don't have to be considered in a prescription and judgment approach ; it allows exploration and discussion within the framework of imperfect tools which are based on simplified modelling and cost accounting which are necessarily conventional. By positioning our reflection around knowledge for the construction of management tools on one hand, and the training of the structures and dependence relations between actors on the other hand, we focus on an approach aiming at collective action and moving closer two conflicting disciplines that are medicine and management. We so confirm our intuition of a medical management control as process of innovative conception which crosses construction of knowledge and relations. This question is all the more relevant as we approach various levels of the organization, the hospital in the strict sense at a "vast hospital".De nombreux travaux ont traité de l'émergence de la fonction contrôle de gestion à l'hôpital notamment depuis la mise en œuvre de la Tarification à l'Activité (T2A). Pour autant, le contrôle de gestion semble souffrir d'un certain nombre d'écueils parmi lesquels une prolifération d'outils essentiellement budgétaires sans lien avec les logiques d'acteurs et sans impact sur les comportements organisationnels. Cette vision du contrôle de gestion nous semble d'autant plus réductrice dans le contexte actuel qu'est celui de la création de la Direction Générale de l'Offre de Soins (DGOS) qui s'inscrit pleinement dans la nouvelle gouvernance du système de santé avec la création des Agences Régionales de Santé (ARS). Elle traduit la volonté affirmée d'avoir une approche globale de l'offre de soins, intégrant aussi bien la ville que l'hôpital. La littérature en management fait état d'un contrôle de gestion qui passe progressivement de l'allocation des ressources au pilotage de la performance dans une perspective stratégique. Cette thèse analyse les insuffisances des outils traditionnels issus de la comptabilité analytique hospitalière dans le contexte d'une déstabilisation de l'objet " hôpital ". Elle repose sur l'hypothèse selon laquelle l'émergence d'un contrôle de gestion médicalisé s'apparente à une démarche de co-conception qui lie la construction de l'instrumentation avec celle du niveau de l'organisation. La réflexion est menée autour de trois terrains sur le modèle de la recherche intervention classés en fonction de deux variables : le degré d'innovation organisationnelle d'une part, caractérisé par des modes de coordination nouveaux renforcés, et l'éloignement par rapport à l'instrumentation traditionnelle du contrôle de gestion d'autre part. Pour autant, même si nous adoptons une démarche plus ambitieuse tournée vers la prise en compte des dynamiques organisationnelles qui se complexifient du fait de l'ouverture de l'hôpital " hors les murs ", nous ne prétendons pas remettre en cause le déploiement et l'utilisation de la comptabilité analytique dans les établissements hospitaliers. Les outils qui en découlent ne doivent pas être considérés dans une optique de prescription et de jugement. Ils doivent davantage être considérés dans une perspective d'exploration et de discussion dans le cadre d'outils imparfaits car reposant sur des modélisations simplifiées et une comptabilité analytique au caractère conventionnel. En positionnant notre réflexion autour des savoirs nécessaires à la construction d'outils de gestion d'une part, et la formation des structures et des relations de dépendance ou de complémentarité qui se créent entre les acteurs d'autre part, nous privilégions une approche qui vise l'action collective condition nécessaire pour rapprocher deux disciplines a priori antagonistes que sont médecine et gestion. Nous confirmons ainsi notre intuition d'un contrôle de gestion médicalisé en tant que processus de conception innovante qui passe par la construction conjointe des savoirs et des relations. Cette question est d'autant plus pertinente que nous abordons différents niveaux de l'organisation, de l'hôpital au sens strict à l' " hôpital étendu "
