Tacrolimus is highly effective in preventing acute rejection after renal transplantation, but displays a narrow therapeutic index and high inter-individual pharmacokinetic variations. As Tacrolimus is a substrate of CYP3A and ABCB1, the effect of potentially relevant genetic polymorphisms, CYP3A5 6986A>G and ABCB1 3435C>T, in both donors and recipients on Tacrolimus pharmacokinetics and clinical outcome was investigated in 209 kidney transplant patients.
