The importance of hematopoietic cells cytomorphology study in the diagnosis of myelodysplastic syndromes.

Zespoły mielodysplastyczne stanowią niejednorodną grupę nowotworów mieloidalnych. Charakteryzują się zaburzoną hematopoezą prowadzącą do cytopenii obwodowej, która jest przyczyną objawów klinicznych. Wielokrotnie modyfikowana, a obecnie obowiązująca od 2008 r. klasyfikacja MDS wg WHO, obejmuje udział procentowy komórek blastycznych oraz zmiany morfologiczne w poszczególnych liniach hematopoezy. W diagnostyce MDS cechą różnicującą jest obecność pierścieniowatych syderoblastów, a podstawowym badaniem jest mikroskopowa ocena cytomorfologii komórek hematopoetycznych szpiku kostnego. Ponadto Międzynarodowy System Oceny Prognostycznej pozwala na prognozowanie przebiegu klinicznego na podstawie kariotypu, liczby cytopenii oraz odsetka procentowego blastów. Analizę cech dysplazji erytroblastów, granulopoezy oraz megakariopoezy przeprowadzono wraz z wynikami badania syderoblastów pierścieniowatych oraz morfologii krwi obwodowej, leukogramu i mielogramu u 10 chorych. Obserwowano takie cechy dyserytropoezy jak: dojrzewanie megaloidalne, mostki międzyjądrowe i cytoplazmatyczne, fragmentacja i hiperlobulacja jądra komórkowego oraz formy wielojądrowe. W cytoplazmie widoczne było nakrapianie zasadochłonne, ciałka Howella Jolly’ego oraz wakuolizacja. Dysgranulopoeza dotyczyła jądra komórkowego, m.in.: hiposegmentacja oraz hipersegmentacja. Występowały także hipogranularne neutrofile i makropolicyty. Obserwowano również asynchronię w dojrzewaniu jądra i cytoplazmy.W układzie płytkotworzenia występowały mikromegakariocyty - patognomoniczna cecha MDS, megakariocyty z hipolobulacją jądra oraz formy olbrzymie, wielojądrowe.Określenie poszczególnych nieprawidłowości morfologicznych w komórkach hematopoetycznych nie daje możliwości diagnostyki różnicowej, jednak jest podstawą do spełnienia ilościowego kryterium ≥10% dysplastycznych komórek z co najmniej 1 linii w rozpoznaniu MDS.Myelodysplastic syndromes constitute a heterogeneous group of myeloid neoplasms. They are characterized by impaired hematopoiesis leading to peripheral cytopenia, which is the cause of clinical symptoms. Modified many times, currently in force since 2008 WHO classification of MDS includes a percentage of blasts and morphological changes in individual hematopoietic lineages.In the diagnosis of MDS, the distinguishing feature is the presence of ringed sideroblasts, and the basic examination is the microscopic evaluation of cytomorphology of bone marrow hematopoietic cells.In addition, the Revised International Prognostic Scoring System allows prediction of the clinical course, based on the karyotype, the number of cytopenias and the percentage of blasts.The features of erythroblast dysplasia, granulopoiesis, and megakaryopoiesis were analyzed with the results of ring sideroblasts and peripheral blood count, leukogram, and myelogram in 10 patients. Features of dyserythropoiesis, such as megaloid maturation, internuclear and cytoplasmic bridges, fragmentation, hyperlobulation of the cell nucleus and multi-nuclear forms were observed. The basophilic stippling, Howell-Jolly bodies and vacuolization were visible in the cytoplasm.Dysgranulopoiesis concerned the cell nucleus: hyposegmentation and hypersegmentation. They were also hypogranular neutrophils and macropolycytes. Asynchrony in maturation of nucleus and cytoplasm were also observed.Micromegakaryocytes – a pathognomonic feature for MDS, megakaryocytes with hypolobulation of the nucleus and giant, multi-nuclear forms was present in the megakaryocytes lineage.Determination of individual morphologic abnormalities in hematopoietic cells do not provide differential diagnosis, but it is the basis for meeting the quantitative criterion of ≥10% dysplastic cells from at least 1 lineage in the diagnosis of MDS
