IMPLEMENTASI KAIZEN DI CV. FERTILINDO AGROLESTARI MOJOSARI

This study was conducted to determine the defective product based on organic fertilizer. Based on these problems then be made to this research by using SIX SIGMA were then carried out by means of continuous improvement Kaizen implementation in the form of KAIZEN FiveStep Plan, and the Five-M Checklist. Data from this study were drawn based on the number of defects during the month of January 2014 - June 2014 obtained the total production in January 2014 amounted to 437 220 kg to 95 700 kg total defect. After processing the data obtained DPMO value of 7291 in January 2014 from a total of 95 700 kg defect. The company is located at 3.1 sigma level with CTQ (Critical To Quality) most defects that cause Over Size. From the analysis it can be concluded that the main cause of disability is a human factor, machinery and materials by means of the implementation of kaizen, the main policies that should be run by the companies that control or tighter controls
