Molecular mechanism in the progression of kidney interstitial fibrosis

La fibrosis intersticial del riñón está asociada a las enfermedades glomerulares crónicas y la nefrotoxicidad farmacológica, esencialmente por cic1osporina A y tacrolimus en pacientes trasplantados, y constituye el factor más importante en el desarrollo de la insuficiencia renal progresiva que acompaña a las enfermedades renales. Aunque el aumento intrarrenal de endotelina I es conocido que provoca isquemia crónica que puede ser importante en el desarrollo de la fibrosis intersticial, otros factores de crecimiento como el factor de crecimiento transformante beta (TGF-â) y mediadores vasculares como la angiotensina II han sido implicados en el proceso. En este artículo se revisa la histología normal del intersticio renal y el papel que juegan la lesión tubular y de la matriz extracelular en la génesis de la fibrosis intersticial del riñón así como su regulación por mecanismos paracrinos y autocrinos vinculados a las endotelinas y TGF-â.Intersticial renal fibrosis is associated to chronic glomerular diseases and nephrotoxicity caused by drugs, essentially cyclosporin A and Tacrolimus (FK506) in trasplanted patients and it is the most important factor by progressive renal insufficiency. Although increased endothelin (Et) is Known to induce chronic ischemia, and may be important in the genesis of interstitial fibrosis, other mediators of fibrogenesis have been implicated, eg, transforming growth factor beta (TGF-â) and angiotensin Il. In this article we review the normal histology of renal intertitium and the role of tubular lesion and extracellular matrix in the genesis of renal fibrosis.
