Influence of inhalation device, active substance and drug formulation on the compliance of patients with obstructive pulmonary diseases

Introduction Obstructive airway diseases, Chronic Obstructive Pulmonary Disease (COPD) and Asthma, are increasing in frequency throughout the world, having a significant impact on quality of life. Compliance plays a major role in disease outcome, as it has been associated with increased morbidity and mortality. A better understanding of the factors associated with adherence, can lead to more appropriate decisions regarding management.MethodsWe surveyed physicians’ perspective on the impact of device and substance characteristics on patients’ compliance. This study was carried out by running a structured questionnaire, to a total of 144 physicians, conducting personal interviews and evaluating answers on a scale from 1 for most to 6 for least important influencing parameter, during a national Respiratory Congress. Evaluated parameters included rapid onset of action, duration of action, ease of use, low flow resistance, ability of the device to confirm the dose, type of inhalation device, and the formulation of the drug. Participants were asked to determine the importance of the above-mentioned parameters in chronic obstructive lung diseases in general and subsequently in COPD and asthma separately. ResultsOn average “Rapid onset of action” was ranked lower (more important- influential) than all the other parameters. It was statistically significantly lower than all other parameters except that of “Type of inhalation device”. On the contrary, “Adverse events” was ranked statistically significantly higher (less important- influential) than all the other parameters.In more detailed analysis, in patients with Asthma, “Rapid onset of action” was ranked statistically significantly lower (more important-influential) than all the other parameters. The next most influential parameters were “Ease of use” and “Duration of action” that were ranked statistically significantly lower (more important- influential) than the other 4 parameters. On the contrary, “The formulation of the drug” was ranked statistically significantly higher (less important) than all other parameters. While in patients with COPD, “Rapid onset of action”, “Duration of action” and “Ease of use” were ranked statistically significantly lower (more important-influential) than the other four parameters. The parameter “formulation of the drug” was ranked statistically significantly higher (less important) than all other parameters, except that of “Ability of the device to confirm the dose”. In comparison of parameters influencing the compliance between patients with COPD and asthma , the following parameters were differentiated in the comparison between patients with Asthma and COPD patients,: Rapid onset of action was ranked statistically significantly lower (more important-influential) in Asthmatic patients, Low flow resistance was ranked statistically significantly higher (less important-influential) in Asthmatic patients, duration of action and ease of use were marginally statistically significantly higher (less important-influential) in Asthmatic patients .DiscussionIn this study surveying the opinion of physicians treating patients with chronic obstructive airways diseases (asthma and COPD), rapid onset of action, ease of use and duration of action were the most important parameters favoring patients’ compliance for both diseases.As expected, rapid onset of action was the most important factor influencing compliance both in COPD and asthma. Interestingly, it was statistically more important in asthma than in COPD. This finding reflects the difference in pathophysiology and clinical behavior between asthma in COPD as perceived by physicians. In general, asthma is considered a disease which is more likely to be complicated with episodes of acute shortness of breath in need of immediate relief. Duration of action was also considered as an important parameter influencing compliance, irrespective of the underlying diagnosis. This parameter was numerically more important in COPD compared to asthma. Duration of action is clinically relevant for bronchodilators as their duration of action has a direct impact on patients’ symptoms and therefore compliance. The above physicians views also correlate with patients’ perspectives.11 The fact that adverse events were not considered at all as an important compliance factor, confirms the strong belief of physicians in the safety of inhaled medications. Ease of use was also considered as a key factor for compliance. In asthma ease of use is especially important during an acute event of dyspnea needing immediate relief. In COPD patients, who are usually older with several comorbidities ease of use is also an understandable parameter that can affect compliance.The increased number of inhaler devices in recent years has resulted in a confusing number of choices for physicians and patients. The selection of an inhalation device for patients with asthma and COPD has become a complicated issue as several parameters should be taken into account, such as: device/drug availability, clinical setting, patient age, the ability to use the selected device correctly, device use with multiple medications, cost and reimbursement, drug administration time, convenience in both outpatient and inpatient settings, and physician and patient preference. Unfortunately, such studies are scarce in the literature. Strengths of this study are the detailed questionnaire, and the relatively high number of respondents. However, we acknowledge certain limitations of the study. First, we recognize that we explored a complex issue which is difficult to approach by "one- shot strong and direct answers” as the answers do not consider the individualization of the patient path, the level of functional severity, the different phenotypes, the age of the patient and the patient's history. Another limitation is that this study exclusively included specialists and lacks the views of general practitioners. Thus, the results cannot be generalized for non-specialists. Furthermore, the study reflects the beliefs of the physicians of a single country and comparative studies among different countries could provide significant insights. Given the limited number of relevant studies, the present work adds to our understanding of physicians’ perspectives on patient compliance.ConclusionIn conclusion, rapid onset of action, ease of use and duration of action, were classified higher among the parameters examined. Further studies are needed among physicians of different specialties and nationalities to identify more rigorously factors increasing patient compliance. Evidence-based guidelines for the selection of the appropriate inhalation device in specific clinical settings are needed.Εισαγωγή Η παρατηρούμενη αυξητική τάση των αποφρακτικών παθήσεων των αεραγωγών, όπως είναι το άσθμα και η Χρόνια Αποφρακτική Πνευμονοπάθεια (Χ.Α.Π.) σε όλο τον κόσμο, έχει ως αποτέλεσμα μια σημαντική επίπτωση στην καθημερινή ποιότητα ζωής του πληθυσμού. Καθώς έχει επίσης αποδειχθεί ότι η συμμόρφωση των ασθενών διαδραματίζει ένα σημαντικό ρόλο στη νοσηρότητα και θνησιμότητα του πληθυσμού, η μεγαλύτερη κατανόηση των παραμέτρων επιρεασμού της θα μπορούσε να οδηγήσει σε καλύτερες και πιο αποτελεσματικές αποφάσεις για τη διαχείριση των αποφρακτικών νοσημάτων. ΜέθοδοιΣτην παρούσα μελέτη , αναζητήθηκε η άποψη θεραπόντων ιατρών σχετικά με το ρόλο της εισπνευστικής συσκευής και χαρακτηριστικών του φαρμάκου στη συμμόρφωση των ασθενών. Η μελέτη ανέλυσε τις απόψεις από 144 συμμετέχοντες , μετά από προσωπικές συνεντεύξεις στο πλάισιο ενός πανελληνίου συνεδρίου. Ειδικότερα ζητήθηκε η αξιολόγηση συγκεκριμένων παραμέτρων (άμεση έναρξη & διάρκεια δράσης, ευκολία χρήσης χαμηλές αντιστάσεις ροής, δυνατότητα επιβεβαίωσης δόσης, τύπος εισπνευστικής συσκευής και το είδος της φαρμακοτεχνικής μορφής) με κλίμακα αξιολόγησης από 1(περισσότερο) έως 6 (λιγότερο) για ασθενείς με αποφρακτικά νοσήματα (γενικά), αλλά και ειδικότερα για ασθενείς με άσθμα κι Χ.Α.Π. Αποτελέσματα Στην παρούσα μελέτη βρέθηκε ότι η «ταχύτητα έναρξης δράσης» αξιολογήθηκε ως η πιο σημαντική ανάμεσα στις εξεταζόμενες παραμέτρους. Το αποτέλεσμα ήταν στατιστικά σημαντικό σε σχέση με τις υπόλοιπες παραμ’ετρους με εξαίρεση τον «τύπο της συσκευής». Παράλληλα, η παράμετρος «ανεπιθύμητες ενέργειες» αποτυπώθηκε ως η λιγότερο σημαντική. Ειδικότερα σε ασθενείς με άσθμα, η «ταχύτητα έναρξης δράσης», αξιολογήθηκε ως πιο σημαντική με την «ευκολία χρήσης» και τη «διάρκεια δράσης» να ακολουθούν σε σημαντικότητα. Στην ομάδα αυτή των ασθενών , η «φαρμακοτεχνική μορφή» κρίθηκε η λιγότερο σημαντική παράμετρος επηρεασμού. Αντίστοιχα στην ομάδα ασθενών με Χ.Α.Π., η «άμεση έναρξη δράσης», η «διάρκεια δράσης» και η «ευκολία χρήσης», αξιολογήθηκαν σημαντικές, ενώ ακολουθεί σε σημαντικότητα και η παράμετρος «δυνατότητα επιβεβαίωσης της δόσης». ΣυζήτησηΌπως ήταν αναμενόμενο η « η ταχύτητα έναρξης δράσης», ήταν η παράμετρος με το μεγαλύτερο βαθμό επηρεασμού στη συμμόρφωση τόσο των ασθματικών όσο και των ασθενών με Χ.Α.Π. και αυτό λόγω της παθοφυσιολογίας των αποφρακτικών νόσων και των εμφανιζόμενων κλινικών συμπτωμάτων δύσπνοιας. Παράλληλα η «διάρκεια δράσης» , θεωρήθηκε επίσης ως σημαντική παράμετρος επηρεασμού και για τις δύο παθήσεις , αν και ήταν ιδιαίτερα σημαντική στην ομάδα των ασθενών με Χ.Α.Π., συγκριτικά με την ομάδα του άσθματος. Ταυτόχρονα η παράμετρος «ανεπιθύμητες ενέργειες» δε θεωρήθηκε ως σημαντική παράμετρος επηρεασμού καθώς οι ιατροί φαίνονται πεπεισμένοι για την ασφάλεια των εισπνεόμενων φαρμάκων. Σχετικά με την παράμετρο «ευκολία χρήσης» , φάνηκε περισσότερο η σημαντικότητα λόγω της ανάγκης για την άμεση χρήση σε περίπτωση ασθματικής κρίσης στους ασθενείς με άσθμα , ενώ στους ασθενείς με Χ.Α.Π. αποτυπώνεται στην δυνατότητα της χρήσης από ασθενείς μεγαλύτερης ηλικίας και με πιθανές συνυπάρχουσες παθήσεις. Καθώς η αυξανόμενη χρήση των εισπνευστικών συσκευών δίνει τη δυνατότητα επιλογών στους θεράποντες ιατρούς , η επιλογή είναι μια απόφαση που πρέπει να λάβει υπ’ όψη της πολλές παραμέτρους , όπως τη διαθεσιμότητα του ιδιοσκευάσματος στην αγορά , η ηλικία των ασθενών , η ικανότητα της σωστής χρήσης μια συσκευής αλλά και την προτίμηση των ασθενών. Σήμερα δεν υπάρχουν αρκετές μελέτες που να αναλύουν τη σχέση της συμμόρφωσης των ασθενών με τους προαναφερόμενους παράγοντες.Στην παρούσα μελέτη θα πρέπει να αναφερθεί η αδυναμία της ανάλυσης των παραμέτρων επηρεασμού της συμμόρφωσης καθώς δε λαμβάνεται υπόψη το προφίλ των ασθενών , οι διαφορετικοί φαινότυποι , η ηλικία και το ιστορικό τους , όπως επίσης ότι και δεν απεικονίζεται (με στατιστική σημαντικότητα) η άποψη των παθολόγων / γενικών ιατρών , αν και συμμετέχουν στην αντιμετώπιση των αποφρακτικών νοσημάτων , λόγω του μικρού αριθμού του δείγματος στη μελέτη. ΣυμπέρασμαΣυμπερασματικά αναλύθηκαν οι απόψεις πνευμονολόγων μεταξύ άλλων οι παράμετροι συμμόρφωσης «άμεση έναρξη δράσης, ευκολία στη χρήση της συσκευής & διάρκεια δράσης του φαρμάκου» και αποδείχθηκε η σημαντική τους επίδραση στη συμμόρφωση των αποφρακτικών ασθενών. Προτείνεται η διενέργεια περισσοτέρων μελετών μελέτες που θα εμπλέξουν περισσότερες ιατρικές ειδικότητες ενώ τα αποτελέσματα αυτών των μελετών θα οδηγήσουν στη σύνθεση κατευθυντήριων θεραπευτικών οδηγιών για την επιλογή της καταλληλότερης συσκευής ανάλογα με το προφίλ του αποφρακτικού ασθενή
