Assessment of the functional limitations caused by musculoskeletal disorders in the upper extremities using the quick-dash questionnaire

Objetivo: comparar la limitación funcional y su evolución entre pacientes con Síndrome de Túnel del Carpo (STC) con aquellos que presentan otros Desórdenes Musculoesqueléticos (DME) en miembros superiores. Materiales y Métodos: estudio descriptivo, longitudinal. Se incluyeron 247 pacientes con estudio electrofisiológico sospechando STC, se aplicó el cuestionario Quick-DASH, módulo de Discapacidad/Síntomas (MDS) y módulo de Trabajo (MT). Se formaron 3 grupos: (1) solo STC, (2) DME con estudio electrofisiológico normal y (3) STC+DME. Para cada grupo se calculó el puntaje Quick-DASH; se realizó seguimiento a un año a 72 pacientes. Resultados: los grupos 2 y 3 mostraron puntajes significativamente más altos en el MDS y el MT (p=0,007; p=0,000). El puntaje del MDS y del MT en pacientes con enfermedad profesional, fue significativamente más alto que el de pacientes con patología de origen común (p=0,000; p=0,000). El grupo de origen profesional mostró puntajes de discapacidad más altos en los pacientes con STC+DME, en los dos módulos (p=0,065 MDS; p=0,004 MT). El seguimiento a un año reportó una evolución más favorable en el grupo 1. Conclusiones: los pacientes con STC, presentan menor discapacidad y dolor y una mejor evolución que aquellos con otros DME de miembros superiores (tenosinovitis de muñeca, epicondilitis medial y/o lateral o tendinitis de hombro) asociados o no con STC. La comorbilidad del STC con otro DME está asociada con mayor limitación funcional. Los pacientes con patologías de origen profesional manifiestan mayor porcentaje de discapacidad que los pacientes con patología de origen común. / Abstract. Objective: to compare the disability and its evolution in patients with the Carpal Tunnel Syndrome (CTS) with those who show other Musculoskeletal Disorders (MSD) in the upper extremities. Materials and Methods: longitudinal descriptive study. Two hundred forty-seven patients were included with an electrophysiological study under suspicion of CTS. The Quick-DASH questionnaire, Disability/Symptoms Module (DSM) and Working Module (WM) were applied. Three groups were made: (1) only CTS, (2) MSD with a normal electrophysiological study and (3), CTS+MSD. Each group got the Quick-DASH score calculated. Seventy-two of the patients were monitored for a year. Results: groups 2 and 3 showed scores significantly higher in DSM and WM (p=0,007; p=0,000). The DSM and WM scores in patients with professional disease were significantly higher than in patients with common origin pathology (p=0,000; p=0,000). The group of professional origin showed disability scores higher than patients with CTS+MSD in the two modules (p=0,065 MSD; p=0,004 WM). The year monitoring reported a favorable evolution in group 1. Conclusions: patients with Carpal Tunnel Syndrome, have less disability and pain and better outcome than those with other upper limb musculoskeletal disorder (tenosynovitis of the wrist, medial and/or lateral epicondylitis or shoulder tendinitis) associated or not with CTS. Comorbidity of Carpal Tunnel Syndrome with another musculoskeletal disorder is associated with greater functional limitations.
