The value of hepatobiliary phases in morphofunctional magnetic resonance imaging in the detection and characterization of focal hepatic lesions

Na busca desta correta caracterização, houve a introdução recente na prática clínica do ácido gadoxético como agente de contraste hepatobiliar- específico (AHBE) em ressonância magnética (RM). Objetivo: Demonstrar o valor das fases hepatobiliares utilizando-se o ácido gadoxético como AHBE em RM na detecção e caracterização de lesões hepáticas focais benignas e malignas. Material e métodos: Foram estudadas retrospectivamente 302 lesões em 136 pacientes que realizaram exames de RM utilizando o ácido gadoxético como AHBE na avaliação de lesões hepáticas focais, sendo 160 lesões benignas (90 hiperplasias nodulares focais, 36 cistos, 22 hemangiomas e 12 adenomas) e 142 malignas (87 metástases e 55 carcinomas hepatocelulares). Dois radiologistas independentemente revisaram em cinco etapas exames de RM, chegando-se posteriormente a consenso nos casos discordantes, categorizando-os em uma escala de 6 pontos, desde escore 0- lesão não detectada, 1 (definitivamente benigno) até 5 (definitivamente maligno). As etapas foram: etapa 1- imagens sem contraste, etapa 2- imagens sem contraste e fases dinâmicas após contraste, etapa 3: fase hepatobiliar após 10 minutos em adição à etapa 2; etapa 4: fase hepatobiliar após 20 minutos em adição à etapa 2; etapa 5: sequências ponderadas em difusão em adição à etapa 3. Resultados: A concordância interobservador foi elevada (coeficiente de concordância de Kappa ponderado entre 0,81 e 1) em todas as etapas na detecção e caracterização das lesões hepáticas focais benignas e malignas. A acurácia ponderada (Az) diagnóstica foi aumentada pela adição das fases dinâmicas: Az ponderada na etapa 1 foi de 0,80 e na etapa 2 foi 0,90. A adição da fase hepatobiliar aumentou a Az para 0,98 (etapa 3), mantendo-se 0,98 nas etapas 4 e 5. O índice de desempenho diagnóstico foi menor na etapa 1 (60,3%), aumentando para 77,8% na etapa 2 e para 94% com a adição da fase hepatobiliar após dez minutos (etapa 3), mantendo-se de 94% com a adição da etapa 4 e com as sequências ponderadas em difusão (etapa 5). Conclusão: As sequências hepatobiliares aumentaram a detecção e confiabilidade diagnóstica na caracterização de lesões hepáticas focais malignas; aumentaram a confiabilidade diagnóstica na caracterização de lesões hepáticas focais benignas; e melhoraram o desempenho diagnóstico na diferenciação entre lesões hepáticas focais benignas e malignas.Abstract: Introduction: The characterization of hepatic focal lesions can be a diagnostic challenge, with high therapeutic and prognostic relevance. In the search for the correct characterization, gadoxetic acid was recently introduced as the hepatobiliary-specific contrast agent in magnetic resonance imaging (MRI) in the clinical practice. Objective: Demonstration of the value of hepatobiliary phases using gadoxetic acid as hepatobiliary-specific contrast agent in MRI in the detection and characterization of benign and malignant focal liver lesions. Material and methods: A total of 302 lesions were retrospectively studied in 136 patients who underwent MR imaging using gadoxetic acid as hepatobiliary-specific contrast agent in the evaluation of focal liver lesions, with 160 benign lesions (90 focal nodular hyperplasias, 36 cysts, 22 hemangiomas, and 12 adenomas), and 87 metastases and 55 hepatocellular carcinomas. Two radiologists performed independent reviews on the MRI scans in five stages, and later discussed discordant cases in order to reach a consensus. The cases were ranked on a 6-point scale, where 0 was considered as no lesion being detected, 1 (definitely benign) to 5 (definitely malignant). The images were analyzed in the following stages: stage 1 – images with no contrast; stage 2 – images with no contrast and dynamic phases after contrast; stage 3 - hepatobiliary phase 10 minutes after stage 2; stage 4 – hepatobiliary phase 20 minutes after stage 2; stage 5 – diffusion-weighted images in addition to stage 3. Results: Inter-observer agreement was high (weighted Kappa coefficient between 0.81 and 1) at all stages in the detection and characterization of benign and malignant focal liver lesions. Accuracy (Az) in stage 1 was 0.80, and 0.90 in stage 2. Weighted accuracy (Az) was increased to 0.98 (stage 3) with the addition of the hepatobiliary phase, remaining at 0.98 in stages 4 and 5. The diagnostic performance index was lower in stage 1 (60.3%), increasing to 77.8% in stage 2, and to 94% with the addition of the hepatobiliary phase after ten minutes (stage 3), remaining at 94% with the addition of stage 4 and diffusion-weighted images (stage 5). Conclusion: Hepatobiliary sequences increased the diagnostic detection and reliability in the characterization of malignant focal liver lesions; it also increased the diagnostic reliability in the characterization of benign focal liver lesions and improved diagnostic performance in the differentiation between benign and malignant focal liver lesions.MestradoFisiopatologia CirúrgicaMestre em Ciência
