A 38-year-old Albanian gentleman presented with a 16-year history of paroxysmal abdominal pain. He describes a dull ache starting in his left iliac fossa moving to the right upper quadrant and into his chest, within 24 h the pain intensifies restricting mobilization and he is confined to bed. The episodes last 3–4 days followed by complete resolution. He has had six admissions in the previous year with multiple negative computed tomography scans of his abdomen and pulmonary arteries. He was documented to have fever on several occasions with raised inflammatory markers (see Figure 1 )
