An Integrity Perspective on the Decentralization of the Health Sector in Colombia

This study analyzes the decentralization of the health sector in Colombia by exploring the characteristics of Institutional Development and Community Participation and their relationship with the socio-economic conditions of Colombian municipalities, as well as the administrative and political issues that affect the implementation processes at the municipal level. The analysis is framed in the context of health sector reform, and decentralization is understood as a public policy which interacts strongly with the political, social, economic and administrative environments. The methodology involves qualitative and quantitative methods; three case studies and a survey of 91 municipalities were carried out. The findings from different sources and methods suggest that the level of progress in the health sector is not consistent with the socio-economic categories in which the municipalities have been classified. This is closely related to the quality of the staff working within the health sector, which is, in turn, connected with the patronage system and the political will of the municipal, regional and national governments.Diese Studie beschäftigt sich mit der Dezentralisierung des Gesundheitssektors in Kolumbien, indem Charakteristika der institutionellen Entwicklung und Gemeindepartizipation und deren Beziehung zu den sozioökonomischen Bedingungen innerhalb der kolumbianischen Stadtverwaltungen sowie administrative und politische Faktoren analysiert werden, die die Implementierung von Gesundheitsdiensten auf der Stadtverwaltungsebene affizieren. Die Studie gründet im Kontext von Gesundheitsreformen, und Dezentralisierung wird als politische Leitlinie verstanden, die aufs Engste mit der jeweiligen politischen, sozialen, ökonomischen und administrativen Einbettung verknüpft ist. Es kamen qualitative und quantitative Verfahren zum Einsatz: drei Einzelfallstudien und ein Survey in insgesamt 91 Stadtverwaltungen. Die entlang dieser unterschiedlichen Datenquellen und Methoden gewonnenen Ergebnisse legen nahe, dass der Grad des Fortschritts im Gesundheitssystem nicht übereinstimmt mit den sozioökonomischen Kategorien, entlang derer die Stadtverwaltungen klassifiziert wurden. Dies ist eng verbunden mit der Qualität des innerhalb des Gesundheitssystems tätigen Personals, die wiederum von der Patronage und dem politischen Willen der jeweiligen regionalen und nationalen Verwaltungen abhängt.Este estudio sobre la descentralización del sector salud en Colombia se centra en el análisis de las características del desarrollo institucional, la participación comunitaria y su relación con las condiciones socioeconómicas de los municipios colombianos, así como de los asuntos administrativos y políticos que afectan el proceso de implementación de la descentralización en el ámbito municipal. El análisis se enmarca en el contexto de la reforma del sector salud y la descentralización se asume como una política pública la cual interactúa fuertemente con el ambiente político, social, económico y administrativo. Se utilizaron métodos cualitativo y cuantitativo, llevando a cabo tres estudios de caso y una encuesta en 91 municipios. Los hallazgos de las diferentes fuentes de información sugieren que el nivel de progreso institucional del sector salud no es consistente con la categoría socioeconómica de los municipios. Lo cual está relacionado con la calidad del personal que trabaja en el sector, lo que, a su vez, está relacionado con el sistema clientelista y la voluntad política de los gobernantes municipales, regionales y nacionales
