This study examined the factors that lead people with schizophrenia to perform poorly on experimental theory of mind (ToM) paradigms. A group with psychosis (n=26) was compared to a group with anxiety disorders (n=27) and a healthy control group (n=25) on two tests of ToM (the Eyes and Hinting tests) and a measure of global empathy (the Empathy Quotient). The psychosis group performed worse than controls on all measures, with negative symptoms as the key negative predictive factor. When divided by remission status, only the non-remitted psychotic group differed from the other two groups on ToM measures.
