Relationship between active contact location and clinical outcome in the treatment of Parkinson's disease with high frequency deep brain stimulation of the subthalamic nucleus

Parkinson’s disease (PD) is a common neurodegenerative pathology that leads to significant impairment of motor function. Medical as well as surgical treatments are available for PD. However, surgical treatment is offered after exhaustion of medical approaches or intolerance to medical treatment. Deep brain stimulation of the subthalamic nucleus (STN-DBS) is one method of surgical treatment that was introduced in the 1990s. STN-DBS results in marked alleviation of selected symptoms and signs associated with the motor disorder. Despite its increasing use as a target, the best area of stimulation as well as the mechanism of action of STN-DBS remains largely unexplored. We conducted a retrospective cohort analysis to correlate patient’s motor outcome and contact location in postoperative imaging. Images were fused to a standard MRI brain template (Colin 27) using the automated non-linear image matching and anatomical labeling (ANIMAL) protocol. Both the fusion techniques and the localization methods were analyzed for validity in a Parkinson’s patient population. Stimulation parameters were incorporated in this analysis using a recently published model. Motor outcome associated with 180 leads was analyzed 6 months following surgery and compared with motor function prior to surgery. There was a statistically significant positive correlation between stimulation location in the lateral and dorsal subthalamic area and motor outcome. Contact localization for optimal motor outcome was shown in the region of the lateral dorsal subthalamic nucleus and zona incerta using a voxel-labeled cytoarchitectonic brain atlas.La maladie de Parkinson est une pathologie neuro-dégénérative commune qui cause de multiples symptômes et un handicap fonctionnel important. Des traitements médicinaux et chirurgicaux sont disponible pour les personnes atteintes de la maladie de Parkinson par contre, les traitements chirurgicaux ne sont offerts qu’après avoir épuisé les traitements médicinaux ou qu’une tolérance à ces derniers ne se développe. La stimulation des noyaux sous-thalamiques (STN-DBS) est une méthode chirurgicale qui a été introduite dans les années 1990. La STN-DBS entraîne une diminution marquée de signes et symptômes prédéfinis associés aux troubles moteurs de la maladie. Malgré l’augmentation de son utilisation comme cible, la meilleure région pour stimuler ainsi que les mécanismes d’action de la STN-DBS restent à être définis. Nous avons fait l’analyse rétrospective d’une cohorte dans le but de corréler l’issue motrice des patients avec la localisation des contacts de stimulation avec l’imagerie postopératoire. Les images ont été fusionnées à un gabarit de cerveau standard (Colin 27) en utilisant le protocole ANIMAL (automated non-linear image matching and anatomical labeling) pour fusion non-linéaire. Les techniques de fusion ainsi que les méthodes de localisation ont été analysées afin d’établir leur validité chez une population de patients ayant la maladie de Parkinson. Les paramètres de stimulation ont été incorporés dans cette analyse à l’aide d’un modèle récemment publié par Bériault et al. L’issue motrice 6 mois postopératoire associée à 180 électrodes à été analysée et comparée à la fonction motrice préopératoire. Une corrélation positive statistiquement significative a été observée entre l’issue motrice et la localisation de la stimulation dans la région latérale et dorsale des noyaux sous-thalamiques. Pour une issue motrice optimale, il a été démontré à l’aide d’un atlas cérébral cytoarchitectonique étiqueté par voxel que les contacts devaient être localisés dans la région latérale-dorsale des noyaux sous-thalamiques et dans la zona incerta
