Osteochondral defects in weight-bearing joints are common, yet difficult to treat. Mosaic Plasty represents a new method of autogenous osteochondral transplantation for the treatment of focal cartilaginous defects secondary to biomechanical chondropathy, traumatic chondral lesions and osteochondritis dissecans of the knee and ankle. The procedure described by Hangody can be performed arthroscopically when lesion does not excee 2 cm in diameter and does not need more then 4-6 grafts. Procedure involves obtaining small cylindrical grafts. Procedure involves obtaining small cylindrical grafts from the non-weight-bearing periphery of the femur at the patelofemoral joint, and transporting them to the prepared defect site. The site can be filled with 8 percent transplanted hyaline cartilage. Surrounding cancelous bed is filled with fibrocartilage. The purpose of this study was to compare our results of mosaicPlasty procedure with non-operative treatment method and to define the indications. MosaicPlasty seems to be an efficient technique to treat osteochondral defects of the knee. In our experience this procedure was successful in 86 percents of the cases. However, the success of this technique depends on many factors: precise selection of the patients, corrective harvest technique and good fit of the graft into defect.
