Taking care of patients with class III malocclusion (mandibular prognathis and maxillary retrognathism) depends on the age of the patient. In extreme case or in case of late diagnosis of the anomaly, the treatment will cover both orthodontic and surgical practice. To reach the optimum result, an orthodontic and surgery symbiosis will be needed as well as motivation from the patient. The orthognathic surgery will be divided in two phases : first a maxillary Lefort I osteotomy then, a mandibular ramus sagittal split osteotomy.
