Orthodontie et chirurgie des classes III squelettiques

Taking care of patients with class III malocclusion (mandibular prognathis and maxillary retrognathism) depends on the age of the patient. In extreme case or in case of late diagnosis of the anomaly, the treatment will cover both orthodontic and surgical practice. To reach the optimum result, an orthodontic and surgery symbiosis will be needed as well as motivation from the patient. The orthognathic surgery will be divided in two phases : first a maxillary Lefort I osteotomy then, a mandibular ramus sagittal split osteotomy. Regarding the orthodontic phase, pre surgery time will be needed to allow the orthodontist to restore a functional occlusion with good canine protection, incisal guide and a stable class I occlusion. Communication between the different protagonists (orthodontist, maxillo-facial surgeon, patient) is key for successful orthodontic and surgical treatment. It is important that this multidisciplinary approach continues with a follow up on results with regards to stability and fonctions amelioration.La prise en charge des patients présentant une classe III squelettique par prognathie mandibulaire et retrognathie maxillaire, varie selon l'âge du patient. Lors de cas extr^mes et d'une détection tardive de l'anomalie, la prise en charge s'orientera vers un traitement à la fois orthodontique et chirurgical. Il sera important d'obtenir une symbiose orthodontico-chirurgicale et une motivation suffisante du patient afin d'atteindre un résultat optimal et durable. La chirurgie orthognathique se décomposera en deux temps avec une ostéotomie de type Lefort I au maxillaire et une ostéotomie sagittale des branches montantes à la mandibule. Concernant la phase orthodontique, on aura une phase pré-chirurgicale permettant une correction dento-maxillaire, un nivellement des arcades et une suppression des compensations dento-alvéolaires. La phase post-chirurgicale va, elle, permettre à l'orthodontiste d'affiner l'occlusion fonctionnelle en recréant les protections canines et le guidage incisif ainsi que le rétablissement d'une occlusion de classe I stable. La communication entre les différents protagonistes (orthodontiste, chirurgien maxillo-facial, patient) reste la clef du succès d'un traitement orthodontico-chirurgical.
