Comparison of effectivenesses on treatment of fluticasone propionate alone and in combination with levocetirizine or montelukast in patients with allergic rhinitis

YÖK Tez ID: 294228Bu çalışmanın amacı, alerjik rinitli hastalarda sadece intranazal kortikosteroid kullanımının ve buna Histamin 1 (H1) veya lökotrien antagonisti eklenerek uygulanan tedavi modellerinin etkinliklerini araştırmaktır.Alerjik rinitli 60 erişkin hasta randomize olarak 3 gruba ayrılmıştır. Altı hafta süre ile Grup I'deki 20 hastaya flutikazon propionat (FP) nazal sprey 400 ?g/gün, Grup II'deki 20 hastaya FP (400 ?g/gün) + levosetirizin (LS) (5 mg/gün) ve Grup III'deki 20 hastaya FP (400 ?g/gün)+ montelukast (ML) (10 mg/gün) tedavisi verilmiştir. Tedavi öncesi (TÖ) ve tedavi sonrası (TS) dönemde hastaların rinokonjunktivit semptomları kaydedilmiş ve yaşam kaliteleri RQLQ ve SF-36 ile, burun tıkanıklığı akustik rinometri (AR) ile değerlendirilmiştir.Çalışmamızda her üç tedavi grubunda, TS dönemde rinokonjunktivit semptom skorlarında TÖ döneme göre istatiksel olarak anlamlı düzelme olduğu saptanmıştır ( p<0.05). Grup II (p<0.001) ve Grup III'ün TS rinit semptom skorlarının (p=0.028) Grup I'den istatistiksel olarak anlamlı şekilde daha düşük olduğu bulunmuştur. SF-36'daki parametrelerinde daha sınırlı düzelme olmakla birlikte; akustik rinometri ölçümlerinde FP tedavisinin sadece volüm 2'yi, FP+LS tedavisinin ise MCA2 ve volüm 2'yi arttırdığı bulunmuştur. Her üç grupta RQLQ'nun; faaliyetler, uyku, burun ve göz dışı belirtiler, genel sorunlar, burun ve göz belirtileri parametrelerinde istatistiksel olarak anlamlı düzelme gözlenmiştirSonuç olarak, alerjik rinit tedavisinde üç tedavi protokolünün de yararlı olacağı, ancak burun akıntısı, burun kaşıntısı, hapşırık gibi semptomları ön planda olan olgularda kombinasyon tedavilerinin tercih edilmesi önerilmektedir.Anahtar Sözcükler: Alerjik rinit, akustik rinometri, flutikazon propionat, levosetirizin, montelukast, RQLQ, SF-36, yaşam kalitesi.The aim of this study is to investigate the the effectiveness of treatment models of intranasal corticosteroid usage alone or combining with Histamine 1 (H1) or with leukotriene antagonists in the patients with allergic rhinitis.60 patients with allergic rhinitis were randomizely divided into three groups. For a period of six weeks, Fluticasone propionate (FP) nasal spray was prescribed to 20 patients in group I with the dosage of 400 ?g/day, FP (400 ?g/day) + levocetirizine (LS) (5mg/day) was prescribed to 20 patients in group II, FP (400 ?g/day) + montelukast (ML) (10mg/day) was prescribed to 20 patients in group III. The patients? rhinoconjunctivitis symptom scores were recorded; and their quality of lives were evaluated with RQLQ and SF- 36 questionnaire, and nasal obstruction was evaluated with acoustic rhinometry (AR) before (BT) and after treatment (AT).In our study, all three treatment groups, statistically significant improvement was detected in rhinoconjunctivitis symptom scores on AT period compared to PT (p <0.05).The rhinitis symptom scores of after treatment period in group II (p<0.001) and group III (p=0.028) were statistically lower than that of group I. Although limited improvement was observed in SF-36'daki parameters; we found that FP treatment increased only the volume 2 parameter, the treatment of FP+ LS increased the MCA2 and Volume 2 parameters in acoustic rhinometry measurements.
