Epilepsiile la copilul cu paralizie cerebrală spastică

Background. Epilepsy, one of the most common brain diseases in children, is a chronic pathology, often associated with Cerebral Palsy (CP). Both are common causes of disability and child mortality. Objective of the study. assessment of the type of epileptic seizures and peculiarities of treatment in children with spastic CP, with the aim of improving the quality of life of these children. Material and Method. Prospective study (2017-2019) on a group of 28 children, aged 2 to 5 years, diagnosed with spastic CP, the forms: bilateral (17 cases – 60.7%), unilateral (11-39.3%), who presented various types of seizures. Children with febrile seizures were excluded from the study. Examinations: electroencephalography (EEG) and brain magnetic resonance imaging (MRI). Results. The following types of seizures were recorded: focal-64.3%, generalized – 28.6%, polymorphic-7.1%. Modify EEG: focal (40.9%), focal with bilateral extension (23.4%); generalized (25%), including hypsarrhythmia (3.6%); mixed (7.1%). MRI: cortical atrophy( 21.4%), cystic formations of various sizes: unilateral (46.4%), bilateral (14.3%); periventricular leukomalacia (10.7%), porencephaly (3.6%), developmental abnormalities of the brain (3.6%). Positive results in treatment by: monotherapy (57.2%), biotherapy (32.1%), polytherapy (10.7%). Conclusion. Epilepsy has an early onset in children with spastic CP, frequently associated with focal seizures. Response to treatment is correlated with the severity of brain damage, often requiring control with one or more preparations. Incorrect treatment adversely affects neurodevelopment. Introducere. Epilepsia, una dintre cele mai răspândite afecţiuni ale creierului la copil, este o patologie cronică, deseori asociată cu paralizia cerebrală (PC). Ambele reprezintă cauze comune de invalidizare şi mortalitate a copiilor. Scopul lucrării. Aprecierea tipului crizelor epileptice și a particularităților tratamentului la copiii cu PC spastică, cu scop de ameliorare a calității vieții acestor copii. Material și Metode. Studiu prospectiv (2017-2019) pe un lot de 28 de copii, cu vârsta între 2 şi 5 ani, diagnosticaţi cu PC spastică, formele: bilaterală (17 cazuri – 60,7%), unilaterală (11 – 39,3%), care au prezentat diverse tipuri de accese convulsive. Au fost excluși din studiu copiii cu convulsii febrile. Examinări: electroencefalografie (EEG) şi imagistice prin rezonanţă magnetică cerebrală (RMN). Rezultate. Au fost înregistrate următoarele tipuri de convulsii: focale – 64,3%, generalizate – 28,6%, polimorfe – 7,1%. Modificări EEG: focale (40,9%), focale cu extindere bilaterală (23,4%); generalizate (25%), inclusiv hipsaritmie (3,6%); mixte (7,1%). RMN: atrofie corticală (21,4%), formațiuni chistice de diverse dimensiuni: unilaterale (46,4%), bilaterale (14,3%); leucomalacie periventriculară (10,7%), porencefalie (3,6%), anomalii de dezvoltare ale creierului (3,6%). Rezultate pozitive în tratament prin: monoterapie (57,2%), bioterapie (32,1%), politerapie (10,7%). Concluzii. Epilepsia are un debut precoce la copiii cu PC spastică, frecvent fiind asociată cu crize focale. Răspunsul la tratament fiind corelat cu severitatea afectării creierului, deseori necesitând control cu unul sau mai multe preparate. Tratamentul incorect influenţează negativ neurodezvoltarea
