Background. Objective of the study. Analyzing a clinical case to elucidate the clinical-evolutionary features of the disease. Material and Methods. A clinical case of bipolar affective disorder has been founded in order to highlight the clinical-evolutionary features of the disease, taking in consideration that it is often misdiagnosed and treated ineffective. Results. Patient A., aged 67 years old, had many hospitalizations in the Clinical Hospital of Psychiatry during a period 2004 -2020. The first case of hospitalization was in 2004, with the diagnosis of “Reactive psychosis,anxious-depressive ". Sarted from year 2005 - 15 hospitalizations have been made with various diagnosis. After 10 years of evolution of the disease, patient had a progress of hypomania which was increased by this period. Anamnestic, catamnestic data, and the current clinical picture were took into attention but before that, test data of PHQ-2 and TAG-2 the diagnosis has been approved as” Affective bipolar disorder. Severe current episode with psychotic symptoms”. Conclusion. Often the diagnosis of bipolar disorder is established late and until then patients receive an average of 3.5 other clinical diagnoses, administering treatment symptomatic. Introducere. TAB, face parte dintr-un grup heterogen de patologii cu simptome clinice similare. Elementul clinic esențial al acestui grup sunt: „modificările dispoziției, acompaniate de un sindrom maniacal/hipomaniacal sau depresiv, complet sau parțial”. Potrivit DSM- V-TR, între 0,5-1,2% din populație suferă de TAB. Scopul lucrării. Anlizând un caz clinic, să elucidăm particularitățile clinicoevolutive ale maladiei. Material și Metode: A fost investigat retrospectiv un caz clinic de tulburare afectivă bipolară în scopul elucidării dificultăților de diagnostic al patologiei respective, datorită faptului că ea, adeseori este incorect diagnosticată și tratată. Rezultate. Pacienta A., vârsta - 67 de ani cu multiple internări în perioada 2004-2020. Internată primar, în anul 2004 cu diagnosticul „Psihoză reactivă, anxios-depresivă”. Din anul 2005, au urmat 15 internări cu diverse diagnostice, dar nu cel de tulburare afectivă bipolară. După 10 ani de evoluţie a maladiei, datorită faptului că pacienta a dezvoltat un episod de hipomanie și, luând în calcul datele catamnestice, tabloul clinic actual, rezultatele testelor psihometrice, s-a stabilit diagnosticul de: tulburare afectivă bipolară. Episod actual sever cu simptome psihotice. Concluzii. Tot mai frecvent, diagnosticul de tulburare afectivă bipolară este stabilit cu întârziere și, până a fi diagnosticați corect, pacienții primesc în medie 3,5 alte diagnostice clinice, administrând tratament simptomatic
