Influence of Holmium Laser Enucleation of the prostate on erectile function: results of a multicentric analysis of 235 patients

La préservation de la fonction érectile (FE) est l’une des priorités de la prise en charge chirurgicale des symptômes du bas appareil urinaire (SBAU) liés à une hypertrophie bénigne de la prostate (HBP). L’objectif de ce travail était de décrire l’évolution de la FE à 3 et 12 mois après énucléation laser de la prostate au laser holmium (HoLEP). Une étude rétrospective multicentrique a été conduite sur 4 centres incluant un total de 235 patients sexuellement actifs avant leur prise en charge chirurgicale et opérés d’un HoLEP entre janvier 2016 et juin 2017. L’évaluation de la FE était faite par auto-questionnaire IIEF-5 à l’inclusion, à 3 mois et à 12 mois postopératoires. L’amélioration ou la dégradation de la FE étaient définies par une variation d’au moins 5 points du score IIEF-5. Résultats : aucune différence significative n'a été observée entre le score IIEF-5 médian préopératoire 19,0 [14,5–23,0] et les scores médians à 3 et 12 mois (18,0 [12,5–23,0], p = 0,15 et 19,0 [14,0–23,0], p = 0,45, respectivement). Trois mois après la chirurgie, 9,8% des patients ont signalé une amélioration de la FE tandis que 15,3% des patients ont signalé une dégradation. Douze mois après la chirurgie, 13,2% des patients ont signalé une amélioration de la FE tandis que 16,2% des patients ont signalé une dégradation. La réduction du score IIEF-5 n'était statistiquement significative que dans le sous-groupe de patients présentant une FE préopératoire normale (IIEF-5 22). Dans ce sous-groupe, 14,9% des patients ont signalé une diminution de plus de cinq points du score total IIEF-5. L'âge et le score ASA élevé étaient significativement associés à une dégradation de la FE 12 mois après la chirurgie. Cette évaluation multicentrique confirme que la fonction érectile n’est pas altérée de manière significative après HoLEP. Néanmoins, 16,2% des patients ont signalé une altération de leur sexualité 12 mois après la chirurgie. Ce pourcentage atteint 14,9% chez les patients ne présentant pas de dysfonction érectile préopératoire. Ces résultats pourraient être pris en compte pour informer les patients en préopératoire des conséquences possibles de la chirurgie.Preserved sexual function is one of the endpoints of the surgical management of lower urinary tract symptoms (LUTS) suggestive of benign prostatic obstruction (BPO). Our aim was to investigate the evolution of erectile function (EF) at 3 and 12 months after holmium laser enucleation of the prostate (HoLEP). Materials A multicentric retrospective study was performed including 235 sexually active patients who underwent HoLEP between January 2016 and June 2017.Evaluation of EF was carried out with the five-item version of the International Index of Erectile Function (IIEF-5) completed before surgery and at 3 and 12 months after surgery. A change of more than five points in either direction in the IIEF-5 score compared to baseline was considered as an improvement or impairment of EF. Results: No significant differences were found between median preoperative IIEF-5 19.0 [14.5–23.0] and median scores at 3 and 12 months (18.0 [12.5–23.0], p = 0.15 and 19.0 [14.0–23.0], p = 0.45, respectively). Three months after surgery, 9.8% of patients reported an improvement, whereas 15.3% of patients reported an impairment. Twelve months after surgery, 13.2% of patients reported an improvement, whereas 16.2 % of patients reported an impairment. The reduction in IIEF-5 score was only statistically significant within the subgroup of patients with normal preoperative EF (IIEF-5 ≥ 22). In this subgroup, 14.9% of patients reported a decrease of more than five points in total IIEF-5 score. Age and high ASA score were significantly associated with impairment of EF 12 months after surgery. In this study, the median IIEF-5 score was not impaired at 3 and 12 months after treatment. Nonetheless, 16.2% of patients reported an impairment of their sexuality. This percentage reached 14.9% for patients who didn’t have any preoperative erectile dysfunction (ED).The results of these figures should be taken into account for preoperative patient counselling concerning the possible consequences of surgery
