Prognostic value of pre-operative CA19-9, neutrophil/lymphocyte and platelet/lymphocyte rates in patients operated for pancreatic ductal adenocarcinoma

Amaç: Günümüzde nötrofil/lenfosit oranı, platelet/lenfosit oranı ve CA 19-9 gibi birçok preoperatif proinflamatuar belirteç çeşitli kanser türlerinin prognozunun tahmininde anlamlı olduklarını bildirmişlerdir. Bu çalışmada amacımız pankreatik duktal adenokarsinom (PDAK) nedeniyle opere edilen hastalarda bu preoperatif parametrelerin değerini araştırmaktır. Materyaller ve Metodlar: Eylül 2011 ile Eylül 2016 tarihleri arasında PDAK nedeniyle opere edilen 118 hastaya ait veriler retrospektif olarak incelendi. Preoperatif dönemde tüm hastaların tam kan sayımlarında; beyaz küre, hemoglobin düzeyi, mutlak nötrofil ve lenfosit sayısı, platelet sayısı, nötrofil/lenfosit oranı (NLO), platelet/lenfosit oranı (PLO) ve CA19-9 düzeyleri kayıt edildi. Ayrıca hastalara ait yaş, cinsiyet, tümör lokalizasyonu, tümör çapı, biliyer dekompresyon yapılıp yapılmadığı, T stage, N stage, evre, perinöral invazyon, metastatik lenf nodu sayısı, cerrahi sınır durumu gibi histolojik özellikler, hasta takip süreleri ve sap kalım süreleri kayıt edildi. Results: Tek değişkenli analiz sonuçlarına göre 4 histopatolojik ve 3 preoperatif serum parametresinin azalmış sağkalım süreleri için prognostic değeri olduğunu belirledik. Kötü diferansiasyon derecesi (p=0.012), perinöral invazyon varlığı (p=0.016), lenf nodu metastaz pozitifliği (p=0.019), cerrahi sınır pozitifliği (p=0.017), yüksek NLO (p=0.001), düşük PLO (p=0.006) ve düşük lenfosit sayımı (p=0.031) olması azalmış prognoz ile ilişkiliydi. Çok değişkenli analiz sonuçlarına göre kötü diferansiason derecesi (p=0.001), tümör çapının  2 cm olması (p=0.025), cerrahi sınırın rezeksiyondan sonra pozitif olması (p=0.014), 74 yüksek NLO (p=0.015) ve düşük PLO (p=0.030) kötü prognoz için bağımsız risk faktörü olduğunu bulduk. Sonuç: Preoperatif serum hematolojik parametrelerden NLO'nun artması ve PLO'nun yüksek olması PDAK nedeniyle rezeksiyon yapılan hastalarda azalmış sağ kalım ile ilişkilidirler ve bağımsız prognostik bir parametre iken CA19-9 ile sağ kalım arasındaki ilişki anlamlı değildir.Aim: Recently, many preoperative proinflammatory markers such as neutrophil / lymphocyte ratio, platelet / lymphocyte ratio and CA 19-9 have been reported to be important in predicting the prognosis of various cancer types. In this study, we aimed to investigate the value of these preoperative parameters in patients undergoing pancreatic ductal adenocarcinoma (PDAC) operation. Materials and Methods: Between September 2011 and September 2016, the data of 118 patients who were operated on due to PDAC were retrospectively reviewed. In the pre-operative period, in the blood counts of all patients; leukocyte count, hemoglobin level, absolute neutrophil count, lymphocyte count, platelet count, neutrophil / lymphocyte ratio (NLR), platelet / lymphocyte ratio (PLR) and CA19-9 levels were recorded. Age, sex, tumor localization, tumor diameter, biliary decompression existence, T stage, N stage, stage, perineural invasion, number of metastatic lymph nodes and histological features such as surgical margin status, follow-up times and survival times regarding patients were recorded. Results: According to univariate analysis results, we determined that 4 histopathological and 3 preoperative serum parameters are prognostic values for reduced survival times. Poorly differentiated grade (p = 0.012), presence of perineural invasion (p = 0.016), lymph node metastasis positivity (0.006), surgical margin positivity (P = 76 0.017), high NLR (p = 0.001), low PLR (p=0.016), and low lymphocyte count (p=0.031), were associated with decreased prognosis. According to the results of multivariate analysis, we found that the poor differentiation grade (p = 0.001), the tumor size  2 cm (p = 0.025), positive surgical margin after resection (p = 0.014), high NLR (p = 0.015) and low PLR (p = 0.030) were independent risk factors for poor prognosis. Conclusion: Increased NLR in the preoperative serum hematologic parameters and high PLR are associated with decreased survival in resected patients due to PDAC and the relationship between CA 19-9, an independent prognostic parameter, and survival is not significant.
