ABSTRACTObjective: To determine if an intensive pharmacist intervention in an automatic therapeutic interchange program affects the frequency of interchange-related problems experienced by patients upon hospital discharge.Methods: In this nonrandomized, prospective pilot evaluation with nonparallel groups, a total of 95 patients were assigned to either the intervention or the control group. Those in the intervention group received information about the automatic therapeutic interchange program, were given the option of using their own medication, and received counselling about discharge medication.
