Suture Versus Vessel Sealer in Abdominal hysterectomy: An Observational study

INTRODUCTION: Hysterectomy is second most common surgery perfomed in women next to cesarean section. Most hysterectomies are performed for benign lesions of uterus/adnexa. Hysterectomy can be performed by vaginal or abdominal route or through laprascopy. AIM OF THE STUDY: To study the safety and efficacy of electrosurgical vessel sealer (Ligasure) in open abdominal hysterectomy compared to hysterectomy done with conventional suture material. METHODOLOGY: This study was conducted in Govt. RSRM Lying in Hospital, Chennai from January 2019 to September 2019.60 cases were studied .Investigations and Preanesthetic evaluation are done for all cases.30 cases underwent Abdominal hysterectomy with Conventional suture and 30 cases underwent Abdominal hysterectomy with ligasure vessel sealer. Outcome of the study measured in terms of Operative time, Blood loss during surgery, complications and postoperative pain. RESULTS: The Operating time in Conventional suture group is 98.5 mins and in vessel sealer group is 84.67mins with p value < 0.001. Thus the operating time in vessel sealer group is lesser compared with operating time in conventional suture group. In study group the mean blood loss is 177.5 ml which is lower when compared to the control group with 215.33 ml blood loss. In control group the mean postoperative pain calculated with visual analogue scale was 7.27 which is lower when compared to the study group mean post operative pain 5.53 on POD-1. In Control group the mean postoperative pain on POD1 and POD2 calculated with visual analogue scale was 2.93and 1.47 which is lower than the mean post operative pain in control group on POD1 and POD2 which was 4.3 and 2.07 respectively. CONCLUSION: The Ligasure vessel sealer usage in abdominal hysterectomy found to decrease the operating time, decrease blood loss during surgery and decrease in postoperative pain compared to hysterectomy done with conventional suture material
